Evil Qi and the Disease Forms of the Zang-Fu

Xié qì zàng fǔ bìng xíng

Chapter Four is the longest treatment of disease in the first third of the Lingshu, and it is really two chapters joined at the waist. The first half asks how evil qi gets in; the second half asks how you know, once it is in, what has happened. Between them stand the five colours, the five pulses, the skin of the cubit, and a catalogue of sixty disease forms. The opening exchange settles the geography. Evil qi strikes a person "high", and the gloss that follows divides the body at the waist: above it evil, below it dampness. But the important sentence is the next one — striking yin, the evil flows to the fu; striking yang, it flows to the channels — because it explains why an illness that begins on the skin so often ends in an organ. The face is where all the yang channels meet, and the chapter gives the routes by which evil entering the face descends: to yangming at the face, taiyang at the nape, shaoyang at the cheek. Evil entering the yin, by contrast, begins at the arm and the shin, where the skin is thin and the flesh moist. The five zang are then wounded in five ways, and the list has the compression of poetry: sorrow and fear wound the heart; cold of body and cold drink wound the lung; falling, and retained bad blood, and great anger wound the liver; being struck down, or drunken entry into the chamber with sweat facing the wind, wounds the spleen; lifting weights, or excess in the chamber, with sweat and bathing, wounds the kidney. Each is a way in which ordinary life damages an organ, and the list was quoted for two thousand years. The second half is diagnostic and numerical. Colour, pulse and the skin of the cubit must agree, as drumstick agrees with drum; to know one is to be a workman, to know two a spirit, to know three both. Then come the correspondences — green with the taut pulse, red with the hooked, yellow with the intermittent, white with the hairy, black with the stony — and the rule that a pulse which conquers the colour means death while one that generates it means recovery. From there the chapter runs through the six pulse qualities in each of the five zang, twelve conditions per organ, sixty in all, and then states how each quality is needled: the urgent deeply and long, the slow shallowly and quickly, the rough by hitting the vessel and pressing the hole afterwards. Two sections of practice follow. The first explains that the springs and transporting points treat the outer channels while the sea points treat the inner fu, and names the sea point of each of the six fu with the posture by which it is taken — Sanli with the foot lowered, Juxu with the foot raised, Weiyang by bending and straightening. The second lists the diseases of the six fu and the point for each, ending with the warning that a needle which misses the qi point and hits the flesh joint lets the evil fight with the true qi and lodge deeper. The received text has its usual crop of old forms. Heat is written with the old character throughout, "candidate" for "sign" appears twice in the old abbreviated shape, the testicles are written with the old form of "gao", the stomach duct with the hand radical, and the sea points of the large and small intestine with "juxu" rather than the later "xu" of the market-place. One phrase has been quietly corrected by every editor: "let what route enter" is written with the character for "command", and read as "now". None of these are scribal errors; they are the orthography of a Ming woodblock, and they are kept.

Huang Di asked Qi Bo: When evil qi strikes a person, how is it? Qi Bo answered: The place where evil qi strikes a person is high.

Huang Di said: Is there a measure to high and low? Qi Bo said: The upper half of the body is where evil strikes; the lower half is where dampness strikes. Therefore it is said that evil qi has no fixed place in striking a person: striking yin, it flows to the fu; striking yang, it flows to the channels.

Huang Di said: Yin and yang differ in name but are the same in kind; above and below they meet, and the channels and network vessels run through one another like a ring without end. When evil strikes a person, now into yin, now into yang, above and below, left and right, without constancy — what is the reason?
Qí Bó yuē: zhū yáng zhī huì, jiē zài yú miàn. zhòng rén yě, fāng chéng xū shí, jí xīn yòng lì, ruò yǐn shí hàn chū, còu lǐ kāi, ér zhòng yú xié. zhòng yú miàn zé xià yáng míng, zhòng yú xiàng zé xià tài yáng, zhòng yú jiá zé xià shào yáng; qí zhòng yú yīng bèi liǎng xié, yì zhòng qí jīng.
Qi Bo said: The meetings of all the yang are on the face. When it strikes a person, it does so at the moment it rides a deficiency, or just after exertion, or when food and drink have brought sweat and the interstices are open, and so it enters as evil. Striking the face, it descends to yangming; striking the nape, it descends to taiyang; striking the cheek, it descends to shaoyang. Where it strikes the chest, back and both flanks, it also strikes the channel concerned.
Huáng Dì yuē: qí zhòng yú yīn nài hé? Qí Bó dá yuē: zhòng yú yīn zhě, cháng cóng bì héng shǐ. fū bì yǔ héng, qí yīn pí bó, qí ròu nào zé, gù jù shòu yú fēng, dú shāng qí yīn.
Huang Di said: What of its striking yin? Qi Bo answered: When it strikes yin it invariably begins at the arm and the shin. For the skin on the yin side of arm and shin is thin and the flesh moist and glossy, and so although both receive wind, it injures only the yin there.
Huáng Dì yuē: cǐ gù shāng qí zàng hū? Qí Bó dá yuē: shēn zhī zhòng yú fēng yě, bù bì dòng zàng, gù xié rù yú yīn jīng, zé qí zàng qì shí, xié qì rù ér bù néng kè, gù hái zhī yú fǔ. gù zhòng yáng zé liū yú jīng, zhòng yīn zé liū yú fǔ.
Huang Di said: Does this then injure the zang? Qi Bo answered: When the body is struck by wind it does not necessarily stir the zang. Therefore when evil enters a yin channel, if the qi of that zang is solid the evil may enter but cannot lodge, and so returns to the fu. Hence striking yang it flows to the channels, striking yin it flows to the fu.
Huáng Dì yuē: xié zhī zhòng rén zàng, nài hé? Qí Bó yuē: chóu yōu kǒng jù zé shāng xīn, xíng hán hán yǐn zé shāng fèi. yǐ qí liǎng hán xiāng gǎn, zhōng wài jiē shāng, gù qì dào ér shàng xíng. yǒu suǒ duò zhuì, è xuè liú nèi, ruò yǒu suǒ dà nù, qì shàng ér bù xià, jī yú xié xià, zé shāng gān. yǒu suǒ jī pū, ruò zuì rù fáng, hàn chū dāng fēng, zé shāng pí. yǒu suǒ yòng lì jǔ zhòng, ruò rù fáng guò dù, hàn chū yù shuǐ, zé shāng shèn.
Huang Di said: What of evil striking a person’s zang? Qi Bo said: Sorrow, worry, fear and dread injure the heart; cold of body and cold drink injure the lung. Because the two colds affect one another, within and without are both injured, and so the qi pathway moves upward. Falling or dropping down leaves bad blood within; or great anger makes the qi rise and not descend, gathering below the flanks, and so injures the liver. Being struck and thrown down, or entering the chamber drunk, with sweat issuing and facing the wind, injures the spleen. Exerting force to lift a weight, or entering the chamber to excess, with sweat issuing and bathing in water, injures the kidney.
Huáng Dì yuē: wǔ zàng zhī zhòng fēng nài hé? Qí Bó yuē: yīn yáng jù gǎn, xié nǎi dé wǎng. Huáng Dì yuē: shàn zāi!
Huang Di said: What of the five zang being struck by wind? Qi Bo said: When yin and yang are both affected, then evil can come and go. Huang Di said: Good.
Huáng Dì wèn yú Qí Bó yuē: shǒu miàn yǔ shēn xíng yě, zhǔ gǔ lián jīn, tóng xuè hé yú qì ěr. tiān hán zé liè dì líng bīng, qí cù hán huò shǒu zú xiè duò, rán ér qí miàn bù yì, hé yě? Qí Bó dá yuē: shí èr jīng mài, sān bǎi liù shí wǔ luò, qí xuè qì jiē shàng yú miàn ér zǒu kōng qiào, qí jīng yáng qì shàng zǒu yú mù ér wéi jīng, qí bié qì zǒu yú ěr ér wéi tīng, qí zōng qì shàng chū yú bí ér wéi xiù, qí zhuó qì chū yú wèi, zǒu chún shé ér wéi wèi. qí qì zhī jīn yè, jiē shàng xūn yú miàn, ér pí yòu hòu, qí ròu jiān, gù tiān qì shèn hán bù néng shèng zhī yě.
Huang Di asked Qi Bo: Head and face with the body and form — they are joined to the bones, linked by the sinews, share the blood and unite in the qi, that is all. When heaven is cold it cracks the earth and piles up ice, and in sudden cold the hands and feet may be slack and useless; yet the face needs no clothing. Why is this? Qi Bo answered: In the twelve channels and the three hundred and sixty-five network vessels the blood and qi all go up to the face and enter the hollow openings. The essence-yang qi goes up to the eyes and makes sight; the separated qi goes to the ears and makes hearing; the ancestral qi goes out at the nose and makes smell; the turbid qi goes out from the stomach to the lips and tongue and makes taste. The fluids of this qi all steam upward to the face, and moreover its skin is thick and its flesh firm, so that the cold of heaven, however severe, cannot overcome it.

Huang Di said: When evil strikes a person, what is the form of the disease? Qi Bo said:

When deficient evil strikes the body, there is shivering and the form is shaken. When upright evil strikes a person it is slight: it first appears in the colour, is not felt in the body, seems present and seems absent, seems gone and seems still there, has form and has no form, and no one knows its true condition. Huang Di said: Good.
Huáng Dì wèn yú Qí Bó yuē: yú wén zhī, jiàn qí sè, zhī qí bìng, mìng yuē míng; àn qí mài, zhī qí bìng, mìng yuē shén; wèn qí bìng, zhī qí chù, mìng yuē gōng. yú yuàn wén jiàn ér zhī zhī, àn ér dé zhī, wèn ér jí zhī, wéi zhī nài hé? Qí Bó dá yuē: fū sè mài yǔ chǐ zhī xiāng yīng yě, rú fú gǔ yǐng xiǎng zhī xiāng yīng yě, bù dé xiāng shī yě. cǐ yì běn mò gēn yè zhī chū hòu yě, gù gēn sǐ zé yè kū yǐ. sè mài xíng ròu, bù dé xiāng shī yě. gù zhī yī zé wéi gōng, zhī èr zé wéi shén, zhī sān zé shén qiě míng yǐ.
Huang Di asked Qi Bo: I have heard that to see the colour and know the disease is called clear-sightedness; to press the pulse and know the disease is called spirit; to ask about the disease and know its place is called workmanship. I wish to hear how seeing and knowing, pressing and obtaining, asking and exhausting it are done. Qi Bo answered: The response of colour and pulse with the cubit is like the response of drumstick and drum, of shadow and echo; they cannot lose one another. This too is the outward evidence of root and branch, of root and leaf: when the root dies the leaves wither. Colour, pulse, form and flesh cannot lose one another. Therefore to know one is to be a workman, to know two is to be a spirit, to know three is to be a spirit and clear-sighted as well.

Huang Di said: I wish to hear it all. Qi Bo answered: When the colour is green the pulse is taut; when red, the pulse is hooked; when yellow, the pulse is intermittent; when white, the pulse is hairy; when black, the pulse is stony. To see the colour and not obtain its pulse, but instead obtain the pulse that conquers it, means death; to obtain the pulse that generates it means the disease will end.

Huáng Dì wèn yú Qí Bó yuē: wǔ zàng zhī suǒ shēng, biàn huà zhī bìng xíng, hé rú?

Qí Bó dá yuē: xiān dìng qí wǔ sè wǔ mài zhī yīng, qí bìng nǎi kě bié yě.
Qi Bo answered: First fix the responses of the five colours and five pulses, and then the disease can be distinguished.
Huáng Dì yuē: sè mài yǐ dìng, bié zhī nài hé?
Huang Di said: Colour and pulse being fixed, how are they distinguished?
Qí Bó yuē: tiáo qí mài zhī huǎn jí, xiǎo dà, huá sè, ér bìng biàn dìng yǐ.
Qi Bo said: Examine the pulse for slow and urgent, small and large, slippery and rough, and the transformation of the disease is fixed.
Huáng Dì yuē: tiáo zhī nài hé? Qí Bó dá yuē: mài jí zhě, chǐ zhī pí fū yì jí; mài huǎn zhě, chǐ zhī pí fū yì huǎn; mài xiǎo zhě, chǐ zhī pí fū yì jiǎn ér shǎo qì; mài dà zhě, chǐ zhī pí fū yì bēn ér qǐ; mài huá zhě, chǐ zhī pí fū yì huá; mài sè zhě, chǐ zhī pí fū yì sè. fán cǐ biàn zhě, yǒu wēi yǒu shèn. gù shàn tiáo chǐ zhě, bù dài yú cùn; shàn tiáo mài zhě, bù dài yú sè. néng cān hé ér xíng zhī zhě, kě yǐ wéi shàng gōng, shàng gōng shí quán jiǔ; xíng èr zhě, wéi zhōng gōng, zhōng gōng shí quán qī; xíng yī zhě, wéi xià gōng, xià gōng shí quán liù.
Huang Di said: How is it examined? Qi Bo answered: When the pulse is urgent the skin of the cubit is urgent too; when the pulse is slow the skin of the cubit is slow too; when the pulse is small the skin of the cubit is reduced and short of qi; when the pulse is large the skin of the cubit rises up; when the pulse is slippery the skin of the cubit is slippery too; when the pulse is rough the skin of the cubit is rough too. Of all these changes some are slight and some extreme. Therefore one who is good at examining the cubit need not wait for the inch, and one who is good at examining the pulse need not wait for the colour. One who can combine them and practise them can be an upper workman, and the upper workman makes whole nine in ten; one who practises two is a middle workman, and the middle workman makes whole seven in ten; one who practises one is a lower workman, and the lower workman makes whole six in ten.

Huang Di said: The six changes of disease — how are they needled? Qi Bo answered: All that are urgent are mostly cold; the slow are mostly hot; the large have much qi and little blood; the small have little blood and qi; the slippery have abundant yang qi with slight heat; the rough have much blood and little qi with slight cold. Therefore for the urgent, insert deeply and retain long; for the slow, insert shallowly and withdraw the needle quickly to remove the heat; for the large, slightly reduce the qi and do not let blood out; for the slippery, withdraw the needle quickly while inserting shallowly, to reduce the yang qi and remove the heat; for the rough, one must hit the vessel and retain the needle long according to its adverse or favorable course. One must first press and follow it, and when the needle is withdrawn press the hole quickly, not letting the blood come out, so as to harmonise the vessel. As for all the small, yin and yang, form and qi are all insufficient: do not take them with the needle, but regulate them with sweet medicine.

Huang Di said: Is there a way in needling these? Qi Bo answered: In needling these one must hit the qi point and not hit the flesh joint. Hitting the qi point, the needle dyes the lane; hitting the flesh joint, the skin hurts. If supplementing and reducing are reversed the disease grows worse. Hitting the sinew, the sinew slackens, the evil qi does not come out but fights with the true qi, chaotic and not leaving, and turns back to lodge within. When needling is not examined, the favorable is taken for the adverse.

Textual notes

Readings follow the base edition; significant variants are recorded here.
