㿉 tuí — the old graph for a swelling of the genitals, later written 㿗; 疝 shàn — a hernia, a mounting pain. Swelling and mounting together.
Shuowen
疝,《說文》作「疝,腹痛也」;㿉即㿗之省,《玉篇》「㿗,陰病也」。「㿉疝」本篇1見,在肝脉;「㿉」字5見,別有「㿉癃」「腸㿉」「内㿉」。

In brief

Hernial swelling of the genitals, given here as what a very slippery liver pulse produces: 滑甚,爲㿉疝. Slipperiness means damp and heat together, and the liver’s channel runs through the genitals, so the two facts are enough for the chapter. The same character returns three times in the spleen’s series with other endings — 㿉癃 for retention, 腸㿉 and 内㿉 for suppurating conditions — so that 㿉 marks the swelling and the second character says where. The chapter is not building a classification; it is naming what each pulse gives, and the names are the ones its readers already used.

The Liver channel of Foot Jueyin arises at the border of the thick hair of the big toe, goes up along the upper border of the instep, one inch from the inner ankle, eight inches above the ankle crosses out behind the taiyin, up the inner border of the popliteal hollow, along the inner thigh, into the pubic hair, past the genitals, to the lower abdomen, beside the stomach, belongs to the liver and connects with the gallbladder, up through the diaphragm, spreading in the flanks and ribs, along behind the throat, up into the nasopharynx, connecting with the eye system, up out of the forehead and meeting the governing vessel at the crown. Its branch goes from the eye system down inside the cheek and around inside the lips. Its branch again separates from the liver, passes through the diaphragm and pours into the lung. When this moves, the disease is pain of the waist, inability to bend or stretch, hernia in men, swelling of the lower abdomen in women, and in severe cases dryness of the throat and a dusty, colourless face. What it generates is fullness of the chest, vomiting and reversal, grain diarrhoea, fox hernia, incontinence of urine, and blocked urination. For all these diseases, reduce where abundant, supplement where deficient, speed the needle where hot, retain it where cold, apply moxa where sunken, and where neither abundant nor deficient take from the channel itself. Where abundant, the inch opening is one times greater than the neck pulse; where deficient, the inch opening is smaller than the neck pulse.

The sinews of foot yangming begin at the middle three toes, knot on the dorsum of the foot, run obliquely outward and upward onto the shin bone, knot at the outer side of the knee, go straight up to knot at the hip joint, ascend along the ribs and belong to the spine. The straight branch ascends along the shin and knots at the knee. A branch knots at the outer shin bone and joins shaoyang. The straight branch ascends along the crouching rabbit, knots at the thigh, gathers at the genitals, spreads over the abdomen, knots at the supraclavicular fossa, ascends the neck, grips the mouth, joins at the cheekbone, knots below at the nose and joins above with taiyang. Taiyang is the upper net of the eye and yangming the lower net. A branch runs from the cheek and knots in front of the ear. Its diseases: the middle toe pulls, the shin cramps, the foot jumps and feels hard, the crouching rabbit cramps, the front of the thigh swells, there is swelling hernia, the abdominal sinews tighten and pull at the supraclavicular fossa and the cheek, and the mouth is suddenly drawn aside. Where it is tight, the eye cannot close; where there is heat, the sinews slacken and the eye cannot open. If the sinews of the cheek are cold they tighten and pull the cheek so that the mouth shifts; if there is heat they slacken and cannot gather, and so the mouth is drawn aside. Treat with horse fat, anointing the tight side, and with white wine mixed with cassia, smearing the slack side; hook it with a mulberry hook, place the ash of burnt mulberry in a pit at the height of the sitting patient, and iron the tight cheek with the ointment. Let him drink good wine and eat roast meat; if he does not drink, let him force himself. Do this with three rubbings, and then treat with heated needle and swift piercing, taking recovery as the measure and the painful spot as the point. It is called the last-spring bi.
