上工
Literal
上 is above, and 工 is the workman — 說文 enters 工 as a carpenter's square, 巧飾, having the skill to adorn, and the medical texts use it for the practitioner. 上工 is therefore not a rank but a claim about when a practitioner acts: chapter 26 defines it by what it treats, the sprout, and by what it knows beforehand, the qi of the three regions and nine indicators. The chapter's whole grading of physicians is compressed into this word and its opposite.
Shuowen

In brief

The physician who acts before the disease has a form. Chapter 26 gives the definition in one sentence: he rescues the sprout, he must see the qi of the three regions and nine indicators first, and he rescues while the qi can still be fully regulated and nothing has yet been ruined. The contrast with 下工 is not about skill with the needle but about when the work begins — and the chapter has just spent a long passage explaining that what the superior physician sees does not appear outwardly at all, which is why everyone else fails to see it.

The superior physician rescues it at the sprout: he must first see the qi of the three regions and nine indicators, and rescue it while it can still be fully regulated and has not yet been ruined. Hence he is called the superior physician.

How it has been translated

Every rendering below is a decision about which reader is being addressed. There is no neutral option.

Wiseman & Fengsuperior physicianWiseman consistently renders 工 as physician in these grades, which is right for the medical texts even though the graph means workman.
Unschuldthe good physicianUnschuld reads 上 as a moral judgement rather than a grade, which softens the technical contrast with 下工.
This sitethe superior physicianThe pair 上工/下工 is a grading of when one acts, not of character, so we keep 'superior/inferior' rather than 'good/bad'. We translate 工 as physician here because the chapter is talking about what such a person knows and when he intervenes.
