Literal
Water (水) plus 干. 《說文》 is blunt about it: 人液也 — the fluid of a person. 'Sweat of labour' 汗顏 etc. later; the word itself names a fluid and nothing about how it came out.
Shuowen

In brief

Chapter 21 hands out five sweats and gives each a different viscus to come from: eating to repletion sweats the stomach, fright that seizes the essence sweats the heart, carrying weight on a long road sweats the kidney, running in fear sweats the liver, labour sweats the spleen. The move is diagnostic rather than decorative — each sweat reports which store has been drawn on. Later chapters take sweat as an ending rather than a message: the final sweat that precedes death in the channel endings, and the drenching sweat that accompanies a lung pulse so dissipated that its colour stays bright.

Gù yǐn shí bǎo shèn, hàn chū yú wèi. Jīng ér duó jīng, hàn chū yú xīn. Chí zhòng yuǎn xíng, hàn chū yú shèn. Jí zǒu kǒng jù, hàn chū yú gān. Yáo tǐ láo kǔ, hàn chū yú pí.
Thus when food and drink are taken to excess, the sweat comes from the stomach; when fright seizes the essence, the sweat comes from the heart; bearing a heavy load on a long road, the sweat comes from the kidney; running in fear, the sweat comes from the liver; toiling with the body in labour, the sweat comes from the spleen.

In a year of water in surplus, the qi of cold spreads and moves and the evil harms the fire of the heart. The people are ill with heat of the body and fretting of the heart, restlessness and palpitations, yin counterflow, cold struck through the upper and the lower, delirious speech and pain in the heart; the cold qi arrives early. It answers above to Mercury. If it is severe the abdomen is enlarged and the shins swell, there is panting and coughing, sweating in sleep and hatred of wind. Great rain comes, dust and mist are dim and pent; it answers above to Saturn. If greater yang presides above, there is rain, ice and snow, frost falls out of season, and the qi of dampness alters things. The illness turns to fullness of the abdomen and rumbling in the bowels, sloppy diarrhoea with food undigested, and thirst with a veiled and wandering mind. Where Shenmen is cut off there is death and no cure; it answers above to Mars and Mercury.

Now essence is the root of the body. Hence one who stores essence does not fall ill of warm disease in spring. One who in the summer heat does not sweat forms wind malaria by autumn. This is the pulse method of the healthy person.

If one half of the body sweats and the other does not, the person wastes away on that side. Sweating that meets dampness brings pimples and prickly heat. Fine grain and rich meat suffice to raise a great boil on the foot; one takes it up as though holding an empty vessel. When the sweat of toil meets wind and cold presses in, there is redness of the nose; if it builds up, there are pimples.

When the yin beats and the yang stands apart, it is called having a child. When yin and yang are both depleted and there is flux of the bowel, death follows. Yang added to yin is called sweating. When yin is depleted and yang beats, it is called flooding.

In all cases where the pulse is amiss, feel it: if it is rough, yang qi is in excess; if it is slippery, yin qi is in excess. Yang qi in excess means the body is hot and without sweat; yin qi in excess means copious sweat and a cold body; both yin and yang in excess means no sweat and yet cold.

Therefore wind is the chief of the hundred diseases. When wind-cold lodges in a person it makes the fine body hair stand straight and the skin close so that heat develops; at that point it can be sweated out and dispersed. Or there is impediment, numbness, swelling and pain, and at that point it can be removed by hot compresses, fire cautery and needling.

Qi Bo said: it is the qi of yin and yang, the pattern of high and low, and the difference of the greater and the lesser. The south-east is yang: what is yang sends its essence down, hence the right is hot and the left warm. The north-west is yin: what is yin offers its essence up, hence the left is cold and the right cool. So the land has its high and its low and the qi its warm and its cool: where it is high the qi is cold, where it is low the qi is hot. Therefore those who go to cold and cool places have distension, and those who go to warm and hot places have sores. Purge downward and the distension stops; induce sweat and the sores stop. This is the constant of the opening and closing of the interstices — only the difference of greater and lesser.

How it has been translated

Every rendering below is a decision about which reader is being addressed. There is no neutral option.

Wiseman & FengsweatThey keep 汗 as sweat throughout, including in the compound terms for sweating patterns.
Unschuldsweat / perspirationHe uses perspiration where the passage sounds more general.
This sitesweatThe chapter's interest is in what sweat tells you, and any English word that has been promoted to a clinical term loses that: one treats diaphoresis, but one asks where a sweat came from. 說文's 人液 — the fluid of a person — is the right level of plainness for a fluid that in this chapter is read rather than treated.
