Long Life, Short Life, the Hard and the Soft

Shòu yāo gāng róu

This is the first chapter in the Lingshu to be built as a sequence of questions put to two different masters. Huang Di begins with Shao Shi, whose single answer lays out the yin and yang of the body with the neatness of a diagram, and then turns to Bo Gao, who answers the rest of the chapter in six exchanges. The change of interlocutor is not decorative. Shao Shi speaks of where disease sits and what to needle; Bo Gao speaks of how long a patient will live and how to make him sweat. The first half is a doctrine of correspondences. Within the body yin and yang divide again, so that the five zang are the yin of the interior and the six fu its yang, while the sinews and bones are the yin of the exterior and the skin its yang. Each of the four positions has its own class of point: the ying and shu for the yin of yin, the he for the yang of yang, the jing for the yin of yang, the luo for the yang of yin. The chapter then sorts diseases by the same grid, and arrives at a rule that recurs throughout the book: what has form and does not hurt is yang, what hurts and has no form is yin, and treatment follows the injured side rather than the visible one.The second half is the earliest systematic discussion of constitutional prognosis. Form and qi must support each other, skin and flesh must wrap each other, the blood and vessels must surpass the frame; where they do not, life is short. Bo Gao then gives the signs one by one — slack or taut skin, large or small bones, firm or brittle flesh — and adds the remarkable image of the foundation wall lower than its ground, a man whose constitution is outbuilt by his own dwelling. The passage is remembered for its bluntness: the physician is asked to decide death and life before he touches the patient, not after. The chapter closes with the three changes of needling and with the earliest recipe in the book. For the labourer the cold bi is scorched with fire; for the nobleman it is warmed with a medicated kerchief, and the preparation of that kerchief is given in full — wine, pepper, ginger, cinnamon, silk and cloth, five days and nights in horse-dung smoulder, thirty pressings, thirty wipings. The technical detail is not padding. It marks the point at which the Lingshu ceases to be a manual of points and becomes a record of practice, with its materials, its durations and its insistence that the patient walk about afterwards and keep out of the wind.

Huáng Dì wèn yú Shào Shī yuē: yú wén rén zhī shēng yě, yǒu gāng yǒu róu, yǒu ruò yǒu qiáng, yǒu duǎn yǒu cháng, yǒu yīn yǒu yáng, yuàn wén qí fāng.
Huang Di asked Shao Shi: I have heard that in the constitution of man there is the hard and the soft, the weak and the strong, the short and the tall, the yin and the yang. I should like to hear the method of it.

Shao Shi replied: within yin there is yin, and within yang there is yang. When yin and yang are examined and understood, needling has a method. When the point at which the disease began is known, needling has a principle. Measure the beginnings of the illness with care, let it answer to the seasons, join it within to the five zang and six fu, and without to the sinews, bones and skin. Thus there is yin and yang within, and yin and yang without. Within, the five zang are yin and the six fu are yang; without, the sinews and bones are yin and the skin is yang. Hence it is said: for disease in the yin of yin, needle the ying and shu of the yin channels; for disease in the yang of yang, needle the he of the yang channels; for disease in the yin of yang, needle the jing of the yin channels; for disease in the yang of yin, needle the luo vessels. Hence it is said: disease in yang is called wind, disease in yin is called bi, and when both are diseased it is called wind-bi. Disease that has form but no pain belongs to the yang class; that which has no form but pains belongs to the yin class. Where there is no form but pain, the yang is intact and the yin has been injured: treat the yin quickly and do not attack the yang. Where there is form but no pain, the yin is intact and the yang has been injured: treat the yang quickly and do not attack the yin. When yin and yang both move, now with form and now without, and vexation of the heart is added, it is called yin overcoming its yang. This is neither exterior nor interior, and the form will not last long.

Huang Di asked Bo Gao: I have heard that in disease of form and qi there is a sequence of before and after, and a response of inner and outer. What is it?

Bo Gao replied: wind and cold injure the form; grief, fear, resentment and anger injure the qi. When the qi is injured the zang fall ill; when the zang are cold the form responds; when the form is struck by wind the sinews and vessels respond. This is the mutual response of form and qi, inner and outer.

Bo Gao replied: for a disease of nine days, three treatments end it; for a disease of one month, ten treatments end it. By this the number is reduced or increased according to distance in time. Where bi has long remained and will not leave the body, look at the blood vessels and let out all their blood.

Bo Gao replied: when form and qi support each other there is long life; when they do not, early death. When skin and flesh wrap each other there is long life; when they do not, early death. When the blood, qi, channels and luo surpass the form there is long life; when they do not surpass it, early death.

Huang Di said: what is meant by the slack and the taut of the form?

Bo Gao replied: where the form is full and the skin slack, there is long life; where the form is full and the skin taut, early death. Where the form is full and the pulse large and firm, this is favourable; where the form is full and the pulse small and weak, the qi is in decline, and decline means danger. If the form is full and the cheekbones do not rise, the bones are small, and small bones mean early death. If the form is full and the great flesh is firm and has its divisions, the flesh is firm, and firm flesh means long life. If the form is full and the great flesh has no divisions and is not firm, the flesh is brittle, and brittle flesh means early death. This belongs to the life that Heaven bestows: to establish the form and settle the qi, and to judge long life and short life thereby, one must understand this establishing of form and settling of qi, and only then come before the patient and decide death and life.

Bo Gao replied: take twenty pints of pure wine, one pint of Sichuan pepper, one catty of dried ginger and one catty of cinnamon heart — four things in all — and chew them fine and steep them in the wine. Take one catty of silk floss and four zhang of fine white cloth and put them in the wine as well. Set the vessel in a smoulder of horse dung, cover it, seal and plaster it so that nothing escapes. After five days and five nights take out the cloth and floss and dry them in the sun; when dry, steep them again, until the essence is exhausted. Each steeping must fill out its day; then take them out and dry them. When dry, take the dregs together with the floss, line the cloth double and make a double kerchief six or seven chi long, making six or seven kerchiefs. To use it, roast the kerchief over a fire of living mulberry charcoal and press it on the place where cold bi was needled, so that the heat enters to the seat of the disease. When the kerchief cools, roast it again and press it, thirty times in all. When sweat comes, wipe the body with the kerchief, also thirty times. Then walk about within the room and do not see the wind. At every needling there must be this pressing, and the disease will be cured. This is what is meant by bringing heat within.

Textual notes

Readings follow the base edition; significant variants are recorded here.
