The Five Changes

五變

Wǔ biàn

The question is the oldest and hardest in medicine: why does the same wind sicken one person and leave another untouched? The Emperor puts it plainly, and even wonders whether heaven might make a separate wind for each. Shao Yu's answer is a single image, developed at length: the carpenter and the timber. The axe is ground, the blade whetted, and the wood still resists or yields according to its own grain — hard here, brittle there, thick-barked or thin, sappy or dry. The wind is the blade; the body is the timber. The comparison is worked out in detail. Early-flowering trees are cut down by spring frost; thin-barked ones split in long rain; firm and brittle alike break in a storm. Five kinds of injury are listed, and the chapter turns at once to the human counterpart: what is hurt is always the branch, never the sound trunk, and man's constant illnesses come from the joints, skin and pores that are not firm, where evil can lodge. From here the chapter becomes a diagnostic manual.Five constitutions follow, each with its own question and its own sign: the person prone to wind with soaking sweat, whose flesh is loose; the person prone to consumption and wasting, soft in the five viscera yet hard in temper, whose anger drives qi upward until heat consumes the flesh; the person prone to cold and heat, with small bones and thin arms and a lower face of different colour; the person prone to blockages, with coarse grain and unfirm flesh; and the person prone to accumulations, with thin dry skin and a stomach and intestines that let evil linger. The five are the chapter's title.Modern readers will recognise the five as a first essay in constitutional medicine: the question is no longer what the wind does, but what sort of body receives it, and the answer is read off the surface — flesh, bone, colour, skin. The last exchange lifts the whole into time. Form explains who falls ill; the year explains when. Establish the year, and the phase that is high or low will show whether the disease rises or turns dangerous — and this is what the chapter calls the rule of the five changes. The movement from wood to body to season is continuous, and the same metaphor carries it: grain, joint, phase. The text writes 麤 for 粗, 麽 for 么, and 臗 where received editions read 䐃; each is reproduced as printed.

The Yellow Emperor asked Shao Yu: I have heard that all diseases, when they first arise, must come from wind and rain, cold and heat, following the fine hairs and entering the pores. Some return, some remain, some become wind swelling with sweating, some consumption and wasting, some cold and heat, some lingering wasting, some accumulations. Strange evil overflows and cannot be counted. I should like to hear why. When people are ill at the same time, one suffers this and another that — is it that heaven generates wind for each person? Why are they so different?

Shao Yu said: When heaven generates wind, it does not do so out of favour to the common people. Its course is fair and upright: those who offend against it get it, those who avoid it come to no harm. It does not seek out people; people offend against it themselves.

Shao Yu said: How good the question! Let me compare it to a carpenter.

The carpenter grinds his axe and adze, whets his knife, and cuts timber. Even in wood there is yin and yang, and there is hard and brittle. The hard does not admit the blade; the brittle sheds its bark; and at the joint, the axe is chipped.

Of trees, those that flower early and leaf early meet spring frost and fierce wind, and the flowers fall and the leaves wither. In long drought, the brittle and thin-barked lose sap from their twigs and the leaves wither. In long overcast rain, the thin-barked and sappy have their bark split and soaked. When a sudden storm rises, the firm and the brittle alike break their branches and are injured. With autumn frost and swift wind, the firm and brittle have their roots shaken and their leaves fall. These five each have what injures them — how much more so with man?

The Emperor said: How does man answer to the wood?

Shao Yu answered: What is injured in the wood is always the branch. The branch that is firm and hard is not injured. Man’s constant illnesses likewise come from the joints, skin and pores that are not firm, where evil lodges — and so he is constantly ill.

shào yú dá yuē: ròu bù jiān, còu lǐ shū, zé shàn bìng fēng.
Shao Yu answered: If the flesh is not firm and the pores are loose, he is prone to wind.

Huáng Dì yuē: hé yǐ hòu ròu zhī bù jiān yě?
The Emperor said: How does one diagnose that the flesh is not firm?

Shao Yu answered: The flesh of the popliteal region is not firm and has no grain. As for grain, it is coarse grain; where the grain is coarse and the skin is not dense, the pores are loose. This speaks of the general condition.

shào yú dá yuē: fū róu ruò zhě, bì yǒu gāng qiáng, gāng qiáng duō nù, róu zhě yì shāng yě.
Shao Yu answered: The soft and weak must have something hard and strong, and hardness and strength give much anger; the soft is easily hurt.
Huáng Dì yuē: hé yǐ hòu róu ruò zhī yǔ gāng qiáng?
The Emperor said: How does one diagnose the soft and weak as against the hard and strong?

Shao Yu answered: Such a person has thin skin and eyes that are firm and deep-set, with a long upward thrust. His heart is hard, and hardness gives much anger; anger drives the qi upward, so that it accumulates in the chest and the blood and qi run counter and remain. The skin is stretched and the flesh filled; the blood vessels do not move; it turns to heat, and heat consumes the flesh and skin, and so there is consumption and wasting. This speaks of one who is violently hard in temperament and weak in flesh.

shào yú dá yuē: xiǎo gǔ ruò ròu zhě, shàn bìng hán rè.
Shao Yu answered: One with small bones and weak flesh is prone to cold and heat.

The Emperor said: How does one diagnose the size of the bones, the hardness or brittleness of the flesh, and the unevenness of the colour?

Shao Yu answered: The cheekbone is the root of the bones. If the cheekbone is large the bones are large; if small, the bones are small. If the skin is thin and the flesh has no fullness, the arms weak and yielding, the colour of the lower face endangered and not the same as that of the upper, stained and standing out alone — these are the signs. And where the arm is thin, the marrow is not full, and so he is prone to cold and heat.
Huáng Dì yuē: hé yǐ hòu rén zhī shàn bìng bì zhě?
The Emperor said: How does one diagnose the person prone to blockages?
shào yú dá yuē: cū lǐ ér ròu bù jiān zhě, shàn bìng bì.
Shao Yu answered: One with coarse grain and unfirm flesh is prone to blockages.
Huáng Dì yuē: bì zhī gāo xià yǒu chù hū?

Shao Yu answered: To know whether high or low, look at each part.

The Emperor said: How does one diagnose the person prone to accumulations in the intestines?

Shao Yu answered: The skin is thin and not moist, the flesh unfirm and sodden. In such a case the stomach and intestines are bad, and being bad, evil qi lingers there and accumulations do their damage. Between spleen and stomach cold and warmth are out of order; evil qi arrives little by little, is stored and remains, and a great accumulation arises.
Huáng Dì yuē: yú wén bìng xíng, yǐ zhī zhī yǐ, yuàn wén qí shí.
The Emperor said: I have heard of the forms of disease and know them now. I should like to hear of their times.

Shao Yu answered: First establish the year, and so know the time. When the time is high the disease rises; when low, it is dangerous. Even if there is no sinking, in that year there is a rushing through, and the disease must arise. This is called falling ill according to the form — the rule of the five changes.

Textual notes

Readings follow the base edition; significant variants are recorded here.
