Cold-Damage Fever

Hán rè bìng

The chapter opens with the fevers that come with cold damage, sorted by the layer they occupy — skin, flesh, bone — and each is read from a small set of signs: hair scorched, nose or lips dry as cured meat, sweat absent or pouring. Treatment runs to the luo vessels and to tonifying the taiyin, and the bone case carries a hard prognosis: once the teeth wither there is nothing to do.From there the chapter turns to the neck. The pulsing vessel beside the throat, renying, is the anchor, and the text walks backwards along the neck naming what lies behind it — futu, tianyou, tianzhu — then down to the armpit for tianfu. This is one of the earliest systematic accounts of the window-of-heaven points, and it closes with a formula: these are the five parts of tianyou. Each point is then tied to a sudden condition, and the list reads like an emergency manual: sudden loss of voice, sudden deafness, sudden cramp with dizziness, sudden heat with blood from nose and mouth. Two long paragraphs follow on the face and the eye. The arm yangming and the foot taiyang enter the cheekbone and thread the teeth, so caries of the lower and upper teeth are treated from different channels. The foot yangming runs beside the nose, and the description of the eye system reaching into the brain is the anatomical high point of the chapter. It ends where Chinese ophthalmology always ends, at the canthi: yin and yang qiao cross at the sharp canthus, and the balance of yang and yin decides whether the eyes stay open or close. The last third is the practical core. Heat jue and cold jue, the slack tongue, the shivering jaw, the four seasons' depths of needling, the five parts of the body where an abscess means death, and the rule for stopping excessive sweating by working the opposite polarity. The final paragraph is a warning about needling itself: hit and stay, and the essence leaks; miss and leave, and the qi gathers into an abscess. The text has a few rough places. 眦三 in the list of five parts is emended to 背三; 臂陽明 before 大迎 is kept although the point belongs to foot yangming; and 反者益其 is left as it stands, with the received 反者益甚 noted in the collation.

The arm yangming that enters the cheekbone and runs through the teeth is named Daying; for caries of the lower teeth take it. If the arm is averse to cold, tonify; if not averse to cold, drain. The foot taiyang that enters the cheekbone and runs through the teeth is named Jiaosun; for caries of the upper teeth take it, at the nose and in front of the cheekbone. When the disease is active and the pulse is full, drain; when empty, tonify. One saying: take it outside the nose.

The foot yangming that runs beside the nose into the face is named Xuanlu; it belongs to the mouth and runs opposite to bind the root of the eye. Look for what is excessive and take it, reducing the surplus and increasing the deficient — do the reverse and the disease grows worse. The foot taiyang that pierces the nape and enters the brain properly binds the root of the eye and is named the eye system; for pain of head and eye take it, between the two sinews in the nape, where it divides after entering the brain. Yin qiao and yang qiao cross: yang enters yin, yin leaves yang, and they meet at the sharp canthus. When yang qi is full the eyes stare; when yin qi is full the eyes close.

For heat jue take foot taiyin and shaoyang, retaining the needles. For cold jue take foot yangming and shaoyin on the foot, retaining them. For a slack tongue with drivelling saliva and vexation, take foot shaoyin. For shivering chills, a chattering jaw, no sweat, abdominal distension and vexation, take hand taiyin. To needle the deficient, needle where the qi has departed; to needle the replete, needle where the qi is arriving.

In spring take the luo vessels, in summer the interstices, in autumn the qi mouth, in winter the channel transports. These four seasons each take its season as the standard. The luo treat the skin, the interstices the flesh, the qi mouth the sinews and vessels, the transports the marrow and the five viscera.

The body has five parts: the squatting rabbit is one; the calf is two — the calf is the shank; the back is three; the transports of the five viscera are four; the nape is five. Boils and abscesses in these five parts are fatal.
bìng shǐ shǒu bì zhě, xiān qǔ shǒu yáng míng, tài yīn ér hàn chū; bìng shǐ tóu shǒu zhě, xiān qǔ xiàng tài yáng ér hàn chū; bìng shǐ zú jìng zhě, xiān qǔ zú yáng míng ér hàn chū. bì tài yīn kě hàn chū, zú yáng míng kě hàn chū, gù qǔ yīn ér hàn chū shèn zhě, zhǐ zhī yú yáng; qǔ yáng ér hàn chū shèn zhě, zhǐ zhī yú yīn.
When disease begins in arm or hand, first take hand yangming and taiyin and sweat appears. When it begins in the head, first take the taiyang of the nape and sweat appears. When it begins in the foot and shank, first take foot yangming and sweat appears. Arm taiyin can produce sweat and foot yangming can produce sweat; so when taking yin brings excessive sweating, stop it from the yang; when taking yang brings excessive sweating, stop it from the yin.

The harm in needling: to hit the disease and not withdraw the needle drains the essence; to miss and withdraw it gathers qi. When essence is drained the disease worsens and the patient cowers; when qi is gathered, boils and abscesses form.

Textual notes

Readings follow the base edition; significant variants are recorded here.
