A Discourse on the Forms of Disease

病能論

Bìng tài lùn

Chapter 46 is a chapter of six separate consultations, and its title names what binds them: 病能, the forms a disease takes — 能 read tài, the same word chapter 45 used for the shapes inversion assumes. Nothing here is a system; each exchange turns on a single clinical question, and each is answered from a different direction. The first reads an abscess of the gastric duct off the ren ying pulse at the neck. The second and third ask why the sick cannot rest and why some cannot lie flat, and answer from the stores — a harmed store unsettles a man until his essence finds somewhere to lodge, while the lung, as canopy of the stores, cannot lie flat once lung qi swells the conduit. The fourth is a lesson in reading a pulse against the season: on the right, sunken and tight is simply winter; on the left, floating and slow is the disease, and it is in the kidney. From there the chapter turns to practice. One name, two grades: an abscess still growing is opened with the needle, one where qi is full and blood has gathered is drained with stone — the same disease treated in two ways. Rage and mania are born in yang when yang qi is abruptly broken and cannot resolve; starve the patient and give him iron filings, for food feeds yang. Wind from wine is met with ze xie, zhu and mi xian in a three-finger pinch after the meal. The chapter closes by glossing the books it has cited — Shang jing, Xia jing, Jin gui, Kui duo, Qi heng — and by drawing the distinction that gives the last of them its name: the strange disease does not die by the four seasons, the constant one does.

Huáng dì wèn yuē: rén bìng wèi wǎn yōng zhě, zhěn dāng hé rú? Qí bó duì yuē: zhěn cǐ zhě, dāng hòu wèi mài, qí mài dāng chén xì, chén xì zhě qì nì, nì zhě rén yíng shèn shèng, shèn shèng zé rè.
The Emperor asks: when a person has an abscess of the gastric duct, how should one examine him? Qi Bo answers: to examine this one should read the stomach conduit. The conduit should be sunken and fine; sunken and fine means the qi is counterflowing, and where qi counterflows the ren ying pulse is exceedingly full. Exceedingly full is heat.
Rén yíng zhě, wèi mài yě, nì ér shèng, zé rè jù yú wèi kǒu ér bù xíng, gù wèi wǎn wéi yōng yě.
The ren ying pulse is the stomach conduit. When it counterflows and is full, heat gathers at the mouth of the stomach and does not move on; that is why the gastric duct becomes an abscess.
Dì yuē: shàn. Rén yǒu wò ér yǒu suǒ bù ān zhě, hé yě? Qí bó yuē: zàng yǒu suǒ shāng, jí jīng yǒu suǒ zhī jì zé ān, gù rén bù néng xuán qí bìng yě.
The Emperor says: good. Some people lie down and yet cannot settle — why is that? Qi Bo says: where one of the stores has been harmed, the person settles only once the essence has somewhere to lodge; that is why one cannot leave an illness hanging.
Dì yuē: rén zhī bù dé yǎn wò zhě, hé yě? Qí bó yuē: fèi zhě, zàng zhī gài yě. Fèi qì shèng zé mài dà, mài dà zé bù dé yǎn wò. Lùn zài qí héng yīn yáng zhōng.
The Emperor asks: and why can some people not lie flat? Qi Bo says: the lung is the canopy of the stores. When lung qi is exuberant the conduit is large, and a large conduit will not let a person lie flat. The discussion of it is in the Qi heng yin yang.
Dì yuē: yǒu bìng jué zhě, zhěn yòu mài chén ér jǐn, zuǒ mài fú ér chí, bù rán, bìng zhǔ ān zài? Qí bó yuē: dōng zhěn zhī, yòu mài gù dāng chén jǐn, cǐ yìng sì shí; zuǒ mài fú ér chí, cǐ nì sì shí.
The Emperor asks: here is one ill with inversion. On examination the right conduit is sunken and tight and the left is floating and slow. If that is not how it should be, where does the disease reside? Qi Bo says: examined in winter, the right conduit is naturally sunken and tight — this accords with the four seasons. The left conduit floating and slow runs counter to them.
Zài zuǒ dāng zhǔ bìng zài shèn, pō guān zài fèi, dāng yāo tòng yě.
Being on the left, it indicates the disease is in the kidney, and it bears somewhat on the lung as well: there will be pain in the low back.
Dì yuē: hé yǐ yán zhī? Qí bó yuē: shào yīn mài guàn shèn luò fèi, jīn dé fèi mài. Shèn wéi zhī bìng, gù shèn wéi yāo tòng zhī bìng yě.
The Emperor asks: how do you say that? Qi Bo says: the lesser yin conduit penetrates the kidney and networks the lung, and now one finds a lung conduit. The kidney is made ill by it, and so the kidney is the disease of pain in the low back.
Dì yuē: shàn. Yǒu bìng jǐng yōng zhě, huò shí zhì zhī, huò zhēn jiǔ zhì zhī, ér jiē yǐ, qí zhēn ān zài? Qí bó yuē: cǐ tóng míng yì děng zhě yě.
The Emperor says: good. With abscesses of the neck, some are treated with stone and some with needle and moxa, and all recover. Where does the truth of it lie? Qi Bo says: these carry the same name but are of different grade.
Fú yōng qì zhī xī zhě, yí yǐ zhēn kāi chú qù zhī, fú qì shèng xuè jù zhě, yí shí ér xiě zhī. Cǐ suǒ wèi tóng bìng yì zhì yě.
Where the qi of the abscess is still growing, one should open it with the needle and let it out; where qi is full and blood has gathered, one should use stone and drain it. This is what is meant by one disease treated in different ways.
Dì yuē: yǒu bìng nù kuáng zhě, cǐ bìng ān shēng? Qí bó yuē: shēng yú yáng yě.
The Emperor asks: here is one ill with rage and mania — where is this disease born? Qi Bo says: it is born in yang.
Dì yuē: yáng hé yǐ shǐ rén kuáng? Qí bó yuē: yáng qì zhě, yīn bào zhé ér nán jué, gù shàn nù yě, bìng míng yuē yáng jué.
The Emperor asks: how does yang make a person maniac? Qi Bo says: yang qi, abruptly broken and unable to resolve, makes a person prone to rage. The disease is named yang inversion.
Dì yuē: hé yǐ zhī zhī? Qí bó yuē: yáng míng zhě cháng dòng, jù yáng shào yáng bù dòng, bù dòng ér dòng dà jí, cǐ qí hòu yě.
The Emperor asks: how does one know it? Qi Bo says: the yang brightness pulses normally move; the greater yang and lesser yang do not move. When what does not move moves greatly and swiftly, that is the sign of it.
Dì yuē: zhì zhī nài hé? Qí bó yuē: duó qí shí jí yǐ. Fú shí rù yú yīn, zhǎng qì yú yáng. Gù duó qí shí jí yǐ.
The Emperor asks: how is it treated? Qi Bo says: take away his food and it stops. Food enters into yin and makes qi grow in yang; therefore taking away his food stops it.
Shǐ zhī fú yǐ shēng tiě luò wéi yǐn. Fú shēng tiě luò zhě, xià qì jí yě.
Have him take iron filings as a drink. Iron filings bear qi downward swiftly.
Dì yuē: shàn. Yǒu bìng shēn rè jiě duò, hàn chū rú yù, wù fēng shǎo qì. Cǐ wéi hé bìng? Qí bó yuē: bìng míng yuē jiǔ fēng.
The Emperor says: good. Here is one with heat in the body, lax and listless, sweating as though from a bath, averse to wind and short of breath. What disease is this? Qi Bo says: the disease is named wind from wine.
Dì yuē: zhì zhī nài hé? Qí bó yuē: yǐ zé xiè, zhú gè shí fēn, mí xián wǔ fēn, hé yǐ sān zhǐ cuō wéi hòu fàn.
The Emperor asks: how is it treated? Qi Bo says: take ze xie and zhu, ten parts each, and mi xian, five parts; combine them, take a three-finger pinch, and take it after the meal.

What is called deep and fine is that it meets the hand like a needle. Palpate it and feel it: what has gathered is hard, and what is broad is large.
