Huáng dì wèn yuē: chūn mài rú xián, hé rú ér xián?
The Yellow Emperor asks: the spring pulse is like a bowstring — what makes it string-like?
2
Qí bó duì yuē: chūn mài zhě, gān yě. Dōng fāng mù yě, wàn wù zhī suǒ yǐ shǐ shēng yě. Gù qí qì lái, ruǎn ruò qīng xū ér huá, duān zhí yǐ cháng, gù yuē xián. Fǎn cǐ zhě bìng.
Qibo answers: the spring pulse belongs to the liver, the wood of the east, that by which the myriad things first begin to grow. So the qi arrives soft and yielding, light, empty and slippery, straight and long — hence “string.” Whatever is contrary to this is disease.
3
Dì yuē: hé rú ér fǎn? Qí bó yuē: qí qì lái shí ér qiáng, cǐ wèi tài guò, bìng zài wài; qí qì lái bù shí ér wēi, cǐ wèi bù jí, bìng zài zhōng.
“What counts as contrary?” When the qi arrives replete and forceful, this is excess, and the disease is in the exterior. When it arrives not replete but faint, this is deficiency, and the disease is in the interior.
4
Dì yuē: chūn mài tài guò yǔ bù jí, qí bìng jiē hé rú? Qí bó yuē: tài guò zé lìng rén shàn wàng, hū hū xuàn mào ér diān jí; qí bù jí, zé lìng rén xiōng tòng yǐn bèi, xià zé liǎng xié qū mǎn. Dì yuē: shàn.
With excess one becomes forgetful, muddled and dizzy, with disease of the crown of the head. With deficiency the chest aches and pulls through to the back, and lower down the two flanks are full.

Xià mài rú gōu, hé rú ér gōu? Qí bó yuē: xià mài zhě xīn yě, nán fāng huǒ yě, wàn wù zhī suǒ yǐ shèng zhǎng yě, gù qí qì lái shèng qù shuāi, gù yuē gōu, fǎn cǐ zhě bìng.
The summer pulse is like a hook — what makes it hooked? It belongs to the heart, the fire of the south, that by which the myriad things flourish and grow. The qi arrives abundant and departs declining — hence “hook.”
6

Arriving abundant and departing abundant as well is excess, and the disease is in the exterior. Arriving not abundant while the departure is abundant instead is deficiency, and the disease is in the interior.
7
Dì yuē: xià mài tài guò yǔ bù jí, qí bìng jiē hé rú? Qí bó yuē: tài guò zé lìng rén shēn rè ér fū tòng, wéi jìn yín; qí bù jí zé lìng rén fán xīn, shàng xiàn kài tuò, xià wéi qì xiè. Dì yuē: shàn.
Excess brings body heat and pain in the skin, becoming a spreading sore. Deficiency brings vexation of the heart, with coughing and spitting above, and leaking of qi below.

Qiū mài rú fú, hé rú ér fú? Qí bó yuē: qiū mài zhě fèi yě, xī fāng jīn yě, wàn wù zhī suǒ yǐ shōu chéng yě. Gù qí qì lái qīng xū yǐ fú, lái jí qù sàn, gù yuē fú, fǎn cǐ zhě bìng.
The autumn pulse belongs to the lung, the metal of the west, that by which the myriad things are gathered and brought to completion. The qi arrives light and empty, floating; it comes urgently and departs scattering — hence “floating.”
9

Arriving hairy, hard in the centre and empty at both sides, is excess, and the disease is in the exterior. Arriving hairy and faint is deficiency, and the disease is in the interior.
10

Dì yuē: qiū mài tài guò yǔ bù jí, qí bìng jiē hé rú? Qí bó yuē: tài guò zé lìng rén nì qì ér bèi tòng, yùn yùn rán; qí bù jí, zé lìng rén chuǎn, hū xī shǎo qì ér kài, shàng qì xiàn xuè, xià wén bìng yīn.
Excess brings counterflow qi and pain in the back, with a smouldering, oppressed feeling. Deficiency brings panting — breathing with scant qi and coughing; above, the qi shows blood; below, one hears the sound of the disease.
11

“Good. The winter pulse is like a camp — what makes it camp-like?” It belongs to the kidney, the water of the north, that by which the myriad things close and store. The qi arrives sunken and beating against the finger — hence “camp.”
12
Dì yuē: hé rú ér fǎn? Qí bó yuē: qí qì lái rú tán shí zhě, cǐ wèi tài guò, bìng zài wài; qí qù rú shù zhě, cǐ wèi bù jí, bìng zài zhōng.
Arriving like a flicked stone is excess, and the disease is in the exterior. Departing like a rapid count is deficiency, and the disease is in the interior.
13
Dì yuē: dōng mài tài guò yǔ bù jí, qí bìng jiē hé rú? Qí bó yuē: tài guò, zé lìng rén xiè yì, jǐ mài tòng, ér shǎo qì bù yù yán; qí bù jí, zé lìng rén xīn xuán rú bìng jī, miǎo zhōng qīng, jǐ zhōng tòng, shào fù mǎn, xiǎo biàn biàn.
Excess brings lassitude, pain along the spinal vessel, and scant qi with no wish to speak. Deficiency leaves the heart hung up as in the illness of hunger, cold in the hollow below the ribs, pain within the spine, fullness of the lower abdomen, and changed urine.
14
Dì yuē: shàn. Dì yuē: sì shí zhī xù, nì cóng zhī biàn yì yě, rán pí mài dú hé zhǔ? Qí bó yuē: pí mài zhě, tǔ yě, gū zàng yǐ guàn sì bàng zhě yě.
“Good. The order of the four seasons and the variations of following and counterflow are now clear — but what does the spleen pulse alone preside over?” It is earth: the solitary viscus that irrigates the four quarters.
15
Dì yuē: rán zé pí shàn è kě dé jiàn zhī hū? Qí bó yuē: shàn zhě bù kě dé jiàn, è zhě kě jiàn.
Can the spleen’s good and evil states be seen in the pulse? The good cannot be seen; only the evil can.
16
Dì yuē: è zhě hé rú kě jiàn? Qí bó yuē: qí lái rú shuǐ zhī liú zhě, cǐ wèi tài guò, bìng zài wài; rú niǎo zhī huì zhě, cǐ wèi bù jí, bìng zài zhōng.
Arriving like flowing water is excess, and the disease is in the exterior. Arriving like a bird’s beak is deficiency, and the disease is in the interior.
17
Dì yuē: fū zǐ yán pí wéi gū zàng, zhōng yāng tǔ yǐ guàn sì bàng, qí tài guò yǔ bù jí, qí bìng jiē hé rú? Qí bó yuē: tài guò zé lìng rén sì zhī bù jǔ, qí bù jí zé lìng rén jiǔ qiào bù tōng, míng yuē chóng qiáng.
Excess makes the four limbs unable to lift; deficiency makes the nine orifices fail to communicate. It is called doubled rigidity.
Dì jù rán ér qǐ, zài bài ér qǐ shǒu yuē: shàn. Wú dé mài zhī dà yào, tiān xià zhì shù, wǔ sè mài biàn, kuí duó qí héng, dào zài yú yī. Shén zhuǎn bù huí, huí zé bù zhuǎn, nǎi shī qí jī. Zhì shù zhī yào, pò jìn yǐ wēi, zhù zhī yù bǎn, cáng zhī zàng fǔ, měi dàn dú zhī, míng yuē Yù jī.
The Emperor starts up in alarm, bows twice and touches his head to the ground: “Good. I have the great essentials of the pulse — the ultimate numbers under heaven, the changes of the five colours and the pulse, assessment and the anomalous-constant. The way lies in the one. The spirit turns and does not turn back; if it turns back it no longer turns, and the pivot is lost. The essential of the ultimate numbers is immediate and minute: write it on a jade tablet, store it in the storehouse of the viscera, read it every morning, and call it the Jade Mechanism.”

The five viscera receive qi from what generates them, transmit it to what they conquer, lodge qi in what generates them, and die at the hands of what they cannot conquer. When disease is about to kill, it first travels to the quarter the viscus cannot conquer, and then the patient dies. This is qi moving counter to the order — hence death.

The liver receives qi from the heart, transmits to the spleen, lodges qi in the kidney, and dies on reaching the lung. The heart receives from the spleen, transmits to the lung, lodges in the liver, and dies at the kidney. The spleen receives from the lung, transmits to the kidney, lodges in the heart, and dies at the liver. The lung receives from the kidney, transmits to the liver, lodges in the spleen, and dies at the heart. The kidney receives from the liver, transmits to the heart, lodges in the lung, and dies at the spleen. All these are deaths by counterflow. Dividing one day and one night into fifths is how the hour of death is reckoned.

Huáng dì yuē: wǔ zàng xiāng tōng, yí jiē yǒu cì, wǔ zàng yǒu bìng, zé gè chuán qí suǒ shèng. Bù zhì, fǎ sān yuè ruò liù yuè, ruò sān rì ruò liù rì, chuán wǔ zàng ér dāng sǐ. Shì shùn chuán suǒ shèng zhī cì.
The five viscera communicate with one another and their shifts all have an order; when one is diseased it transmits to what it conquers. Left untreated, by rule in three months or six, in three days or six, it passes through all five viscera and death is due. This is transmission following the conquest order.
22
Gù yuē: bié yú yáng zhě, zhī bìng cóng lái; bié yú yīn zhě, zhī sǐ shēng zhī qī, yán zhī zhì qí suǒ kùn ér sǐ.
Thus: he who distinguishes the yang knows whence the disease came; he who distinguishes the yin knows the term of life and death — that is, he knows it will die on reaching the quarter where it is cornered.

Shì gù fēng zhě, bǎi bìng zhī zhǎng yě. Jīn fēng hán kè yú rén, shǐ rén háo máo bì zhí, pí fū bì ér wéi rè, dāng shì zhī shí, kě hàn ér fā yě; huò bì bù rén zhǒng tòng, dāng shì zhī shí, kě tāng yùn jí huǒ jiǔ cì ér qù zhī.
Therefore wind is the chief of the hundred diseases. When wind-cold lodges in a person it makes the fine body hair stand straight and the skin close so that heat develops; at that point it can be sweated out and dispersed. Or there is impediment, numbness, swelling and pain, and at that point it can be removed by hot compresses, fire cautery and needling.

Fú zhì, bìng rù shè yú fèi, míng yuē fèi bì, fā kài shàng qì.
Left untreated, the disease enters and lodges in the lung; it is called lung impediment, and brings on coughing and upward-rushing qi.
25

Left untreated, the lung transmits the disease on to the liver; it is called liver impediment, also named reversal. There is pain in the flanks and the bringing up of food; at that point one may press, or needle.
26

Left untreated, the liver transmits it to the spleen; it is called spleen wind. It erupts as heat, there is heat in the abdomen, vexation of the heart and discharge of yellow. At that point one may press, may give medicine, may bathe.

Left untreated, the spleen transmits it to the kidney; it is called mounting and conglomeration. The lower abdomen is smouldering hot and painful, and there is a white discharge; it is also called gu. At that point one may press and may give medicine.
28

Left untreated, the kidney transmits it to the heart; the sinews and vessels pull against one another and tighten. It is called tetany; at that point one may cauter and may give medicine. Left untreated, at the full ten days death is due by rule. The kidney thus transmits to the heart, and the heart turns back and transmits to the lung, with cold and heat developing — by rule death is due in three years. Such is the order of the disease.
29
Rán qí cù fā zhě, bù bì zhì yú chuán, huò qí chuán huà yǒu bù yǐ cì, bù yǐ cì rù zhě. Yōu kǒng bēi xǐ nù, lìng bù dé yǐ qí cì, gù lìng rén yǒu dà bìng yǐ.
But what erupts suddenly need not be treated by the order of transmission; some transmutations do not follow the sequence, entering out of order. Grief, fear, sorrow, joy and anger prevent it from keeping its order, and so bring on great disease.
Yīn ér xǐ dà xū zé shèn qì chéng yǐ, nù zé gān qì chéng yǐ, bēi zé fèi qì chéng yǐ, kǒng zé pí qì chéng yǐ, yōu zé xīn qì chéng yǐ, cǐ qí dào yě. Gù bìng yǒu wǔ, wǔ wǔ èr shí wǔ biàn, jí qí chuán huà. Chuán chéng zhī míng yě.
Thus great joy leaves a great emptiness and the kidney qi encroaches; anger lets the liver qi encroach; sorrow lets the lung qi encroach; fear lets the spleen qi encroach; anxiety lets the heart qi encroach. Such is its way. So there are five diseases, and five times five — twenty-five variations, through their transmission and transformation. These are the names of transmission and encroachment.

With the same wasting, pain inside the abdomen, unease in the heart, heat in the shoulder, nape and body, the muscle masses broken and flesh gone, the eye sockets sunken — when the true visceral pulse appears and the eyes no longer recognise anyone, death is immediate. In sudden emptiness, striking the body abruptly, the five viscera are cut off and shut, the pulse vessels do not communicate, qi does not come and go — it is like falling or drowning, and no term can be set. The pulse may cease and not come, or beat five or six times to one breath while the flesh has not yet wasted; even if the true visceral pulse is not seen, death still follows.

Qí bó yuē: wǔ zàng zhě, jiē bǐng qì yú wèi, wèi zhě, wǔ zàng zhī běn yě. Zàng qì zhě, bù néng zì zhì yú shǒu tài yīn, bì yīn yú wèi qì, nǎi zhì yú shǒu tài yīn yě.
Qibo answers: the five viscera all receive their qi from the stomach; the stomach is the root of the five viscera. The qi of a viscus cannot reach the hand taiyin by itself — it must go by way of stomach qi before it arrives there. Thus each viscus, in its own season, reaches the hand taiyin by its own act. When evil qi prevails, the essential qi declines. In severe disease the stomach qi cannot travel with it to the hand taiyin, and so the qi of the true viscus appears alone. Appearing alone means the disease has overcome the viscus — hence death. “Good,” says the Emperor.

In treating any disease, examine the form and the qi, the colour and its lustre, the abundance or decline of the pulse, whether the illness is new or longstanding — and then treat. Do not come late to the hour.

When form and qi match, it can be treated. When the colour is lustrous and floating, it will readily resolve. When the pulse follows the four seasons, it can be treated. A pulse that is yielding yet slippery has stomach qi, and is called easy to treat — take it while the season allows.

When form and qi are at odds, it is hard to treat. A withered colour without lustre will hardly resolve. A pulse replete and hard means the disease grows worse. A pulse going against the four seasons cannot be treated. One must assess these four difficulties and state them plainly.
47

Going against the four seasons means getting a lung pulse in spring, a kidney pulse in summer, a heart pulse in autumn, a spleen pulse in winter — and their arrival all hanging, cut off, sunken and rough. When the seasonal form of the viscus is absent — sunken and rough in spring and summer, floating and large in autumn and winter — this too is named going against the four seasons.

A surging pulse, hot skin, swollen abdomen, no passage front or back, oppression and dimmed vision — these are the five repletions. A fine pulse, cold skin, scant qi, draining and discharge front and back, food and drink not entering — these are the five emptinesses.

“Yet some survive at this point — why?” If gruel enters the stomach and the draining stops, the empty one lives. If the body sweats and the bowels move, the replete one lives. These are the signs.
