A Discourse on the Balanced Person and the Pulse Images

平人氣象論

Píng rén qì xiàng lùn

The eighteenth chapter supplies what the seventeenth assumes: a standard. It opens by counting. A balanced person's pulse beats twice on an exhalation and twice on an inhalation, five times to a breath, and the physician uses his own calm breathing as the measure of the patient's. One beat to a breath is scant qi; three, with agitation, is warmth, wind or impediment; four or more, or a pulse that now races and now stalls, is death. From counting the chapter moves to its doctrine, which is the doctrine of stomach qi: the regular qi of a balanced person is received from the stomach, and every seasonal pulse is judged by how much stomach qi it still carries — slightly string-like in spring, slightly hooked in summer, slightly soft in late summer, slightly hair-like in autumn, slightly stone-like in winter. String without stomach qi is death, and so with each of the others; a pulse that carries the quality of the season to come foretells illness when that season arrives. The chapter then gives the great network vessel of the stomach at the breast, a long list of what the inch opening says under the fingers, the true depot pulse with the days on which each depot dies, the signs of water and of jaundice, and the pulse of pregnancy. It closes with one of the best-known passages in the book: for each of the five depots, the balanced pulse, the diseased pulse and the dying pulse, each named by what it is like — a string of pearls, lang-gan jade, falling elm pods, a hen treading the ground, a belt hook, a newly drawn bowstring, a bird's beak, water leaking through a roof, stones struck by a flick of the finger.

Qibo replies: When a person exhales once the pulse moves twice, and when he inhales once it moves twice again; as the breath settles between them it moves once more, so that in one breath the pulse moves five times, with the deep long breath counted in besides — this is named a balanced person. A balanced person is one who is not ill.

cháng yǐ bù bìng tiáo bìng rén, yī bù bìng, gù wéi bìng rén píng xī yǐ tiáo zhī wéi fǎ.
通常以无病之人的呼吸去衡量病人的脉象;医生自己无病,呼吸均匀,所以医生调匀自己的呼吸再去诊察病人之脉,这就是诊脉的法则。
One regularly uses the breathing of one who is not ill to gauge the person who is ill; the physician is not ill, and so he calms his breathing on the patient’s account and uses it as the measure — this is the method.

Now, when the balanced heart pulse comes, it is bead on bead like a string of pearls, like drawing the hand over lang-gan jade — this is called heart balanced, and in summer it takes stomach qi as its root. When the diseased heart pulse comes, it is panting and continuous, with a slight bend within it — this is called heart disease. When the diseased liver pulse comes, it is full, replete and slippery, like drawing the hand along a long pole — this is called liver disease. When the dying liver pulse comes, it is tense and ever stiffer, like a newly drawn bowstring — this is called liver death.

When the balanced spleen pulse comes, it is harmonious, soft and well separated, like a hen treading the ground — this is called spleen balanced, and in late summer it takes stomach qi as its root. When the diseased spleen pulse comes, it is replete, full and rapid, like a hen lifting her foot — this is called spleen disease. When the dying spleen pulse comes, it is sharp and hard, like the pecking of a crow, like the spur of a bird, like water leaking through a roof, like a stream running away — this is called spleen death.

When the balanced kidney pulse comes, it is panting, heaped and hooked, and hard when pressed — this is called kidney balanced, and in winter it takes stomach qi as its root. When the diseased kidney pulse comes, it is like pulling on a vine, and harder still when pressed — this is called kidney disease. When the dying kidney pulse comes, it springs like a rope being snatched away, and beats like stones flicked from the finger — this is called kidney death.
