A Discourse on Contralateral Puncture

Miù cì lùn

Sixty-three is the one the Suwen gives to a single technique. The chapters around it are theory and catalogue; this one is a manual, and it says the same thing twenty times over, once for each channel, because the technique is easier to show than to define. The definition comes first, and it is a distinction rather than a description. Evil that climbs from the skin and the hair through the grandchild vessels into the network vessels and then into the channels ends by harming the five viscera, and that is treated in the channels. But sometimes the evil settles in the grandchild network and stops there, the network closes and will not open, the evil cannot get into the channels, and it floods sideways into the great network instead. What is in the great network does what the channels do not: it crosses. The left pours into the right and the right into the left, it interferes with the channels without entering them, it has no fixed place and never reaches a transport point. That is the disease contralateral puncture is for. The Emperor then asks the obvious question, and the answer is the clearest sentence in the chapter. In the great puncture the evil is in the channel: one side is full, so the other side is diseased, and one must hit the channel itself. In contralateral puncture the evil is in the network, and the pain lies at a place that crosses the channel rather than following it — so one takes the right for a disease of the left. Everything after that is cases, and they are remarkably uniform: a channel, a symptom, a point on a nail a leek-leaf's breadth from the tip, a number of punctures, and how soon it should end. Several of them count their punctures by the moon — one on the first day of the waxing moon, two on the second, fifteen on the fifteenth, fourteen on the sixteenth, rising and then falling — and the chapter explains why: overdo it and the qi is drained, underdo it and the evil does not come out. Two things in it are worth more than their length. The first is the case of the fall: bad blood inside, the belly full, neither urine nor stool passing — for which the text says, before any needling, give a medicine that frees the passages. The second is the closing rule, which is the whole method in three lines: look at the channels first, feel them and follow them, examine vacuity and repletion and regulate; if it will not regulate, puncture the channels; where there is pain and yet the channel is not diseased, puncture contralaterally; and take every blood-filled network you find in the skin regions. The last case in the chapter is the one that shows why the technique matters — the five networks meeting in the ear, a body whose vessels all move while the person knows nothing, a corpse that is not dead, brought back with five punctures, a bamboo tube and a cup of wine.

Huáng dì wèn yuē: yú wén miù cì, wèi dé qí yì, hé wèi miù cì?
The Yellow Emperor asks: I have heard of contralateral puncture, but I have not grasped its meaning. What is contralateral puncture?
Qí bó duì yuē: fú xié zhī kè yú xíng yě, bì xiān shè yú pí máo; liú ér bù qù, rù shè yú sūn mài; liú ér bù qù, rù shè yú luò mài; liú ér bù qù, rù shè yú jīng mài, nèi lián wǔ zàng, sàn yú cháng wèi, yīn yáng jù gǎn, wǔ zàng nǎi shāng, cǐ xié zhī cóng pí máo ér rù, jí yú wǔ zàng zhī cì yě, rú cǐ zé zhì qí jīng yān.
Qi Bo replies: When evil lodges in the form it must first settle in the skin and the down. If it stays and does not leave it enters the grandchild vessel; if it stays and does not leave it enters the network vessel; if it stays and does not leave it enters the channel vessel, links inward to the five viscera and spreads to the stomach and bowels, so that yin and yang are both affected and the five viscera are harmed. This is the order by which evil enters at the skin and the hair and reaches its extreme in the five viscera. In such a case one treats the channels.

But here the evil lodges in the skin and the hair and settles in the grandchild network. It stays and does not leave, the network is blocked and will not open, the evil cannot get into the channels, and it floods into the great network, which gives rise to an unusual disease.

Its qi has no fixed place, and it does not enter the transport points of the channels. This is what is named contralateral puncture.

The Emperor says: I wish to hear about contralateral puncture, taking the right for a disease of the left and the left for a disease of the right. How is it done, and by what does it differ from the great puncture?

Qi Bo says: When evil lodges in the channels, a full left makes the right diseased and a full right makes the left diseased. There are also cases that shift, where the pain on the left has not yet ended and the vessel on the right is already affected. In such a case one must use the great puncture, and one must hit the channel itself, not the network. As for disease of the network, its pain lies at a place that crosses the channel rather than following it, and that is why it is named contralateral puncture.
Dì yuē: yuàn wén miù cì nài hé? Qǔ zhī hé rú?
The Emperor says: I wish to hear how contralateral puncture is carried out, and how the places are chosen.
