Literal
繆 crossed, at odds, out of place (《說文》: 枲之十絜也 — hanks of hemp bound together, and so entangled, crossed, wrong); 刺 to pierce (《說文》: 直傷也). A puncture made across, on the side opposite the disease.
Shuowen

In brief

The needling method named in the chapter’s prescription, and distinct from ordinary needling: the puncture is made on the side opposite the disease, and — in the later chapter devoted to it — in the surface network vessels rather than the channels. Here it is only one item in a sequence: remove the accumulation, move the limbs gently, keep the clothing warm, needle across, and the frame comes back.

Qibo says: level it by weighing. Remove the stasis and cut away the old, stir the four extremities gently, warm the clothing, and needle the place by contralateral puncture, so as to restore the frame. Open the ghost gate, clean the pure storehouse, and the essence will be taken up in season. Once the five yang have been distributed and the five storing organs flushed, the essence generates itself, the frame fills itself, bone and flesh keep one another, and the great qi is at last level.

The classic speaks of the waxing and waning of qi, of its tipping and shifting to left and right, of treating the lower by means of the upper and the right by means of the left, of surplus and insufficiency, and of supplementing and draining at the bloom and transport points. All this I know.

How it has been translated

Every rendering below is a decision about which reader is being addressed. There is no neutral option.

Common Englishcontralateral needling, or opposite-side needlingBoth are accurate and both are widely used.
This sitecontralateral puncture刺 is a puncture, a single act with an implement; needling suggests the later system of acupuncture, which the text does not have. Contralateral is the anatomist's word for 繆 and says exactly what the character says — the puncture goes in across from where the trouble is.
