Huáng dì wèn yuē: yú wén cì fǎ yán, yǒu yú xiè zhī, bù zú bǔ zhī, hé wèi yǒu yú? Hé wèi bù zú?
The Emperor asks: I have heard it said in the rules of needling – where there is excess, drain it; where there is insufficiency, supplement it. What is this excess, and what is this insufficiency?
2
Qí bó duì yuē: yǒu yú yǒu wǔ, bù zú yì yǒu wǔ. Dì yù hé wèn?
Qi Bo answers: there are five excesses, and there are also five insufficiencies. Which of them does the Emperor wish to ask about?
3
Dì yuē: yuàn jìn wén zhī.
The Emperor says: I want to hear them all.

Qí bó yuē: shén yǒu yú, yǒu bù zú; qì yǒu yú, yǒu bù zú; xuè yǒu yú, yǒu bù zú, xíng yǒu yú; yǒu bù zú; zhì yǒu yú, yǒu bù zú, fán cǐ shí zhě, qí qì bù děng yě.
Qi Bo says: the spirit has its excess and its insufficiency; so have the qi, the blood, the body and the will. These ten are not alike in their qi.

Dì yuē: rén yǒu jīng qì jīn yè, sì zhī, jiǔ qiào, wǔ zàng shí liù bù, sān bǎi liù shí wǔ jié, nǎi shēng bǎi bìng, bǎi bìng zhī shēng, jiē yǒu xū shí. Jīn fū zǐ nǎi yán yǒu yú yǒu wǔ, bù zú yì yǒu wǔ, hé yǐ shēng zhī hū?
The Emperor says: a man has essence, qi, fluid and humour; four limbs and nine orifices; five depots and sixteen parts; three hundred and sixty-five joints. From these the hundred diseases arise, and every one of them has its deficiency and its excess. Yet you, master, speak of five excesses and five insufficiencies – how do they come to be?
6
Qí bó yuē: jiē shēng yú wǔ zàng yě.
Qi Bo says: they all arise in the five depots.
7
Fú xīn zàng shén, fèi zàng qì, gān zàng xuè, pí zàng ròu, shèn zàng zhì, ér cǐ chéng xíng.
The heart stores the spirit, the lung the qi, the liver the blood, the spleen the flesh, the kidney the will – and out of these the body is formed.
Zhì yì tōng, nèi lián gǔ suǐ, ér chéng shēn xíng wǔ zàng.
Will and thought pass through and connect inward to the bone and marrow, and so the body and its five depots are completed.
Wǔ zàng zhī dào, jiē chū yú jīng suì, yǐ xíng xuè qì.
The ways of the five depots all come out through the conduits of the channels; it is by them that blood and qi are moved.
10
Xuè qì bù hé, bǎi bìng nǎi biàn huà ér shēng.
When blood and qi are not in harmony, the hundred diseases arise out of that change.
Shì gù shǒu jīng suì yān.
And so the conduits are to be kept and guarded.

Dì yuē: shén yǒu yú bù zú hé rú?
The Emperor asks: what are excess and insufficiency of the spirit like?
Qí bó yuē: shén yǒu yú zé xiào bù xiū, shén bù zú zé bēi. Xuè qì wèi bìng, wǔ zàng ān dìng, xié kè yú xíng, xiǎn xī qǐ yú háo máo, wèi rù yú jīng luò yě, gù mìng yuē shén zhī wēi.
Qi Bo says: when the spirit is in excess a man laughs without stopping; when it is insufficient he grieves. But if blood and qi have not yet pooled and the five depots are settled, the evil only lodges in the body: a shivering rises in the fine hair and has not entered the channels and network vessels. This is called a faint disorder of the spirit.

Dì yuē: bǔ xiè nài hé?
The Emperor asks: how does one supplement and drain it?
15
Qí bó yuē: shén yǒu yú, zé xiè qí xiǎo luò zhī xuè, chū xuè wù zhī shēn chì, wú zhòng qí dà jīng, shén qì nǎi píng. Shén bù zú zhě, shì qí xū luò, àn ér zhì zhī, cì ér lì zhī, wú chū qí xuè, wú xiè qí qì, yǐ tōng qí jīng, shén qì nǎi píng.
Qi Bo says: in excess of the spirit, drain the blood of the small network vessels. Let blood, but do not thrust the needle deep; do not strike the great channel, and the spirit qi will be calm. Where the spirit is insufficient, look for the empty network vessel, press it to bring the qi, then needle it to open it. Let no blood out, let no qi escape, make the channel pass through, and the spirit qi will be calm.
16
Dì yuē: cì wēi nài hé?
The Emperor asks: how is the faint disorder needled?
Qí bó yuē: àn mó wù shì, zhuó zhēn wù chì, yí qì yú bù zú, shén qì nǎi dé fù.
Qi Bo says: massage without letting go; set the needle but do not push it in. Move qi into what is insufficient, and the spirit qi will be restored.
18
Dì yuē: shàn. Yǒu yú bù zú nài hé?
The Emperor says: good. And what of the excess and insufficiency of the qi?
Qí bó yuē: qì yǒu yú zé chuǎn kài shàng qì, bù zú zé xī lì shǎo qì. Xuè qì wèi bìng, wǔ zàng ān dìng, pí fū wēi bìng, mìng yuē bái qì wēi xiè.
Qi Bo says: when the qi is in excess there is panting, coughing and rising qi; when it is insufficient the breathing is free but the qi is scant. If blood and qi have not pooled and the five depots are settled, and it is only the skin that is slightly affected, this is called a faint leak of the white qi.
20
Dì yuē: bǔ xiè nài hé?

Qí bó yuē: qì yǒu yú zé xiè qí jīng suì, wú shāng qí jīng, wú chū qí xuè, wú xiè qí qì. Bù zú zé bǔ qí jīng suì, wú chū qí qì.
Qi Bo says: in excess of qi, drain the conduit of the channel – do not wound the channel, let no blood out, let no qi escape. Where it is insufficient, supplement the conduit, and let no qi go out.
22

Dì yuē: cì wēi nài hé?

Qi Bo says: massage without letting go. As you bring the needle out, look at it and say: I am going to go deeper. When the needle reaches the man he will surely change countenance; the essence qi submits of itself, the evil qi scatters in confusion and has nowhere to rest, and leaks out through the interstices. Then the true qi and the body are at one again.
24
Dì yuē: shàn. Xuè yǒu yú bù zú nài hé?

Qí bó yuē: xuè yǒu yú zé nù, bù zú zé kǒng. Xuè qì wèi bìng, wǔ zàng ān dìng, sūn luò shuǐ yì, zé jīng yǒu liú xuè.
Qi Bo says: when the blood is in excess there is anger; when it is insufficient, fear. If blood and qi have not pooled and the five depots are settled, but the water of the grandchild network vessels overflows, then there is blood retained in the channels.

Qi Bo says: when the body is in excess there is swelling of the belly and the stools and urine do not pass freely; when it is insufficient the four limbs cannot be used. If blood and qi have not pooled and the five depots are settled, and the flesh only faintly stirs, this is called faint wind.

The Emperor says: good. I have heard the forms of deficiency and excess; I do not yet know how they are produced.
Qí bó yuē: qì xuè yǐ bìng, yīn yáng xiāng qīng, qì luàn yú wèi, xuè nì yú jīng, xuè qì lí jū, yī shí yī xū.
Qi Bo says: qi and blood pool together, yin and yang tip against one another, the qi is disordered in the defence and the blood runs counter in the channels. Blood and qi leave the places where they dwell, and one part is full while another is empty.
44
Xuè bìng yú yīn, qì bìng yú yáng, gù wéi jīng kuáng; xuè bìng yú yáng, qì bìng yú yīn, nǎi wéi jiǒng zhōng; xuè bìng yú shàng, qì bìng yú xià, xīn fán wǎn shàn nù; xuè bìng yú xià, qì bìng yú shàng, luàn ér xǐ wàng.
Blood pooling in yin and qi in yang makes fright and madness; blood pooling in yang and qi in yin makes heat in the interior; blood pooling above and qi below makes the heart fret and a man quick to anger; blood pooling below and qi above makes confusion and forgetfulness.
45
Dì yuē: xuè bìng yú yīn, qì bìng yú yáng, rú shì xuè qì lí jū, hé zhě wéi shí? Hé zhě wéi xū?
The Emperor asks: blood pooling in yin and qi in yang – if blood and qi have left their places in this way, which is the excess and which the deficiency?
46
Qí bó yuē: xuè qì zhě, xǐ wēn ér wù hán, hán zé sè bù néng liú, wēn zé xiāo ér qù zhī. Shì gù qì zhī suǒ bìng wéi xuè xū, xuè zhī suǒ bìng wéi qì xū.
Qi Bo says: blood and qi like warmth and hate cold. In the cold they congeal and cannot flow; in warmth they disperse and pass away. So where the qi has pooled, there the blood is empty; where the blood has pooled, there the qi is empty.
47
Dì yuē: rén zhī suǒ yǒu zhě, xuè yǔ qì ěr. Jīn fū zǐ nǎi yán xuè bìng wéi xū, qì bìng wéi xū, shì wú shí hū?
The Emperor says: what a man has is blood and qi, and nothing more. Yet you say that when the blood pools it is deficiency, and when the qi pools it is deficiency – is there then no excess at all?
48
Qí bó yuē: yǒu zhě wéi shí, wú zhě wéi xū, gù qì bìng zé wú xuè, xuè bìng zé wú qì, jīn xuè yǔ qì xiāng shī, gù wéi xū yān.
Qi Bo says: what is there is excess, what is not there is deficiency. So where the qi pools there is no blood, and where the blood pools there is no qi. Now blood and qi have lost one another, and that is deficiency.
49
Luò zhī yǔ sūn mài, jù shū yú jīng, xuè yǔ qì bìng, zé wéi shí yān.
The network vessels and the grandchild vessels both pour into the channels. Blood and qi pooling together – that is excess.
50
Xuè zhī yǔ qì, bìng zǒu yú shàng, zé wéi dà jué, jué zé bào sǐ.
When blood and qi both run upward, there is the great counterflow; and in that counterflow a man dies suddenly.
51
Qì fù fǎn zé shēng, bù fǎn zé sǐ.

Qi Bo says: yin and yang each have their meeting places at the transport points. The yang pours into the yin; the yin fills and goes outward. When yin and yang are evenly balanced they fill out the body, and the nine probes are as one. Such a man is called a balanced man.

What arises in the yang is got from wind, rain, cold and heat. What arises in the yin is got from food and drink, from lodging and place, and from the yin and yang of joy and anger.

Dì yuē: fēng yǔ zhī shāng rén nài hé?
The Emperor asks: how do wind and rain wound a man?
57

Qi Bo says: when wind and rain wound a man they lodge first in the skin, and pass on into the grandchild vessels; when those are full they pass on into the network vessels; when those are full they pour into the great channels. Blood and qi together with the evil lodge in the partings and interstices; the vessel is hard and large, and hence the name, excess. In excess the outside is hard and full: it cannot be pressed, and pressing it hurts.

Qi Bo says: when cold and damp strike a man, the skin slackens and does not draw together, the flesh is hard and tight, the nourishing blood congeals and the defence qi goes away: hence the name, deficiency. In deficiency the skin lies folded and the qi is insufficient; press it and the qi gathers enough to warm the place, so he feels ease and no pain.

Qi Bo says: when joy and anger are not kept in measure, the yin qi rises counterflow; when it rises, the lower part is empty; and when the lower part is empty, the yang qi moves into it. Hence it is called excess.

Qi Bo says: joy makes the qi sink, grief makes the qi disperse, and when the qi disperses the vessels are empty. If to that is added cold food and drink, the cold qi steams and fills the middle; the blood congeals and the qi goes away. Hence it is called deficiency.
64

The Emperor says: the classic says that emptiness of the yang gives cold without, emptiness of the yin gives heat within, fullness of the yang gives heat without, fullness of the yin gives cold within. I have heard this. What I do not know is how it comes to be so.
65

Qi Bo says: the yang receives its qi from the upper burner, and by it warms the skin and the partings of the flesh. Let cold qi be without, and the upper burner will not pass through; when the upper burner does not pass through, the cold qi stays outside alone, and there is shivering.

Qi Bo says: when a man is worn by labour, the qi of the body declines and grows scant and the qi of grain is not abundant; the upper burner does not move and the lower duct does not pass through. The stomach qi heats, the hot qi steams the chest, and there is heat within.
68

Qi Bo says: when the upper burner does not pass freely, the skin is dense and shut, the interstices are closed and blocked, the dark openings do not open, and the defence qi cannot get out and away: hence heat without.

71
Qí bó yuē: jué qì shàng nì, hán qì jī yú xiōng zhōng ér bù xiè, bù xiè zé wēn qì qù, hán dú liú, zé xuè níng sè, níng zé mài bù tōng, qí mài shèng dà yǐ sè, gù zhōng hán.
Qi Bo says: the counterflow qi rises, and cold qi collects in the chest without being drained. Not drained, the warm qi goes away and the cold stays alone; then the blood congeals, and when it congeals the vessels do not pass through. The vessel is full, large and rough: hence cold in the interior.
72

The Emperor asks: when yin and yang have pooled, and blood and qi with them, and the shape of the disease is formed – how is it needled?
73

Qi Bo says: in needling this, take it from the conduits of the channels: take the blood from the construction, take the qi from the defence. Use the body itself as the measure, and let the number of points and their height and depth follow the four seasons.
74

The Emperor asks: blood and qi have pooled, the shape of the disease is formed, yin and yang tip against one another – how does one supplement and drain?
75

Qi Bo says: to drain an excess – when the qi is full, insert the needle. Let the needle go in together with the qi, to open the gate, as one opens a door; let the needle come out together with the qi, and the essence qi is not wounded while the evil qi goes down. Leave the outer gate unclosed so that the disease can get out; enlarge the way by shaking, as one widens a road. This is called the great draining. Press the hole as you come out, and the great qi is brought to heel.
76

Dì yuē: bǔ xū nài hé?

Qi Bo says: hold the needle but do not set it in at once; first settle the man’s mind. Wait for the out-breath and insert: the qi goes out, the needle goes in. The hole of the needle is stopped on all four sides, so the essence has nowhere to escape. When the needle feels full, bring it out quickly: the qi goes in, the needle comes out, and the heat cannot return. Close the gate, let the evil qi scatter abroad, and the essence qi is kept. Move the needle watching for the hour of the qi: the near qi is not lost, and the far qi comes. This is called pursuing it.

The Emperor says: you say the deficiencies and excesses are ten and arise in the five depots – yet the five depots are only five vessels. The twelve channels all give rise to disease, and you speak only of the five depots. The twelve channels all network the three hundred and sixty-five joints; when a joint is diseased the channel must be reached, and the diseases of the channels all have their deficiency and excess. How do these two fit?
Qí bó yuē: wǔ zàng zhě, gù dé liù fǔ yǔ wéi biǎo lǐ, jīng luò zhī jié, gè shēng xū shí.
Qi Bo says: the five depots have the six palaces for their outer and inner from of old; the channels, network vessels, limbs and joints each give rise to their own deficiency and excess.
