A Discourse on Needling Malaria

剌瘧

Cì nüè

Chapter 36 turns the theory of chapter 35 into a prescription pad. Where the treatise asked why the fit keeps time, this chapter asks what the physician is to do about it, and answers with a list: six malarias of the foot channels, six of the stores, and then a set of rules that govern every case. Each entry is given in the same compressed shape — the part that suffers, the feeling that marks it, and the place to put the needle. The six foot-channel malarias run tai yang to jue yin and read almost like a clinic notebook: the back and head, the shivering and the hatred of company, the craving for firelight, the sighing, the vomiting, the fear. The six store malarias name lung, heart, liver, spleen, kidney and stomach, and several of them are recognisable at a glance — the ashen face of liver malaria, the shut door and shuttered window of kidney malaria, the hunger that cannot eat of stomach malaria. Then the chapter turns from the names of illnesses to the signs that decide treatment, and it is here that it says something unusual for a book of needling: when the pulse is slack, large and empty, use medicine and do not needle. Timing governs everything else — treat a meal-time before the fit, for after that the moment is gone. The chapter closes with the rules for what to do when the first, second and third attempts fail, and with the standing instruction to ask where the illness began and needle there first.
