䯒 héng, the upper part of the shin, the shank. A rare graph: the Suwen writes it where other texts write 胻. Chapter 32 uses it twice, both in the kidney heat disease, and both times paired: hot upper body, cold and sore shank. The word matters because the pairing does — the shank is cold while the soles burn.

In brief

The shank — the upper part of the lower leg. Chapter 32’s rare graph for it, 䯒, appears twice, both in the kidney heat disease: first as the site of the opening soreness, then, once the heat contends, as a place that is cold and sore while the soles of the feet are hot. That inversion is the clinical point of the word here, and it is why the chapter names the shank rather than simply saying the leg.

In a year of metal in surplus, the qi of dryness spreads and moves and the wood of the liver receives evil. The people are ill with pain below both flanks and in the lower abdomen, red and painful eyes with sores at the corner of the eye, and ears that hear nothing. If the severity and killing is extreme, the body is heavy and there is freting oppression, pain in the chest that draws to the back, and fullness in both flanks with pain drawing into the lower abdomen; it answers above to Venus. If it is severe there is panting and coughing with counterflow qi, pain in the shoulder and back, and the sacrum, the private parts, the thigh, the knee, the hip, the calf, the shin and the foot are all affected; it answers above to Mars. The qi of gathering is harsh and the qi of growth sinks; plants and trees draw in and the blue-green dries and falls. The illness turns to sudden pain; the flanks cannot be turned or lain upon; the coughing counterflow is severe and blood escapes. How it has been translated

Every rendering below is a decision about which reader is being addressed. There is no neutral option.

Wiseman & FengshinThey use the ordinary anatomical word, which is clear and slightly broader than the Chinese.
Unschuldlower legUnschuld is broader still, which loses the fact that the text names a specific part of the leg.
This siteshankWe keep 'shank', which names the upper part of the lower leg as 䯒 does and is the word English already has for it.
