A Discourse on Strange Diseases

Qí bìng lùn

Ten ailments that do not follow the ordinary patterns are put to Qi Bo in turn: a woman great with child who loses her voice in the ninth month; a fullness under the ribs with counterflowing qi that has not yielded for two or three years; a swelling of hips, thighs and shanks with pain encircling the navel; a rapid chi pulse with the sinews standing out; a headache that has lasted for years; a sweet taste in the mouth and then a bitter one; a urinary block attended at once by five excesses and two insufficiencies; an illness the child is born with; and a kidney wind that will not let its patient eat. Several of the answers turn less on what to do than on what must not be done. The pregnant woman is left alone, because the network vessels of the womb are cut off and will open again in the tenth month. The breath accumulation is given over to guiding-and-pulling rather than the needle. The submerged beam must not be stirred, for stirring it turns it into a disease of blocked urine. The chapter gives the classic account of how rich and sweet food makes internal heat and centre fullness and turns into wasting-and-thirsting, to be treated with lan, and it closes the case of the five excesses and two insufficiencies with the plain verdict that a body which is neither surface nor interior is already lost.

Huáng dì wèn yuē: rén yǒu chóng shēn, jiǔ yuè ér yīn, cǐ wéi hé yě? Qí bó duì yuē: bāo zhī luò mài jué yě.
The Emperor asks: a woman is heavy with child and, in the ninth month, loses her voice. What is this? Qi Bo answers: the network vessels of the womb are cut off.
Dì yuē: hé yǐ yán zhī? Qí bó yuē: bāo luò zhě, xì yú shèn, shào yīn zhī mài, guàn shèn xì shé běn, gù bù néng yán.
Why do you say so? Qi Bo answers: the network of the womb is tied to the kidney; the lesser yin vessel penetrates the kidney and ties to the root of the tongue, and therefore she cannot speak.
Dì yuē: zhì zhī nài hé? Qí bó yuē: wú zhì yě, dāng shí yuè fù. Cì fǎ yuē: wú sǔn bù zú, yì yǒu yú, yǐ chéng qí zhěn, rán hòu tiáo zhī.
How is it treated? Qi Bo answers: it is not treated; in the tenth month it returns of itself. The Canon of Needling says: do not detract from what is insufficient and do not add to what is in excess, so as to bring an enduring illness into being, and regulate it only afterwards.
Suǒ wèi wú sǔn bù zú zhě, shēn léi shòu, wú yòng chán shí yě.
By “do not detract from what is insufficient”: when the body is wasted and thin, the piercing stone is not to be used.
Wú yì qí yǒu yú zhě, fù zhōng yǒu xíng ér xiè zhī, xiè zhī zé jīng chū, ér bìng dú shàn zhōng, gù yuē zhěn chéng yě.
By “do not add to what is in excess”: there is a formation in the abdomen, and to drain it is to let the essence out while the illness alone takes hold of the interior. Hence it is said that an enduring illness has been formed.
Dì yuē: bìng xié xià mǎn, qì nì, èr sān suì bù yǐ, shì wéi hé bìng? Qí bó yuē: bìng míng yuē xī jī. Cǐ bù fáng yú shí, bù kě jiǔ cì, jī wéi dǎo yǐn fú yào, yào bù néng dú zhì yě.
The Emperor asks: an illness of fullness below the ribs, with counterflowing qi, that has not let up for two or three years — what illness is this? Qi Bo answers: the illness is named breath accumulation. It does not interfere with eating. It must not be cauterised or needled; for accumulation one uses guiding-and-pulling and takes medicines, for medicine alone cannot cure it.
Dì yuē: rén yǒu shēn tǐ bì gǔ héng jiē zhǒng, huán qí ér tòng, shì wéi hé bìng? Qí bó yuē: bìng míng yuē fú liáng, cǐ fēng gēn yě.
The Emperor asks: a person whose body, hips, thighs and shanks are all swollen, with pain encircling the navel — what illness is this? Qi Bo answers: the illness is named submerged beam. This is a root of wind.
Qí qì yì yú dà cháng, ér zhuó yú huāng, huāng zhī yuán zài qí xià, gù huán qí ér tòng yě. Bù kě dòng zhī, dòng zhī wéi shuǐ niào sè zhī bìng yě.
Its qi overflows into the large intestine and lodges in the huang, and the source of the huang lies below the navel, hence the pain encircling the navel. It must not be stirred; to stir it is to make it a disease of scant and hesitant urination.
Dì yuē: rén yǒu chǐ mài shuò shèn, jīn jí ér xiàn, cǐ wéi hé bìng? Qí bó yuē: cǐ suǒ wèi zhěn jīn. Shì rén fù bì jí, bái sè hēi sè xiàn zé bìng shèn.
The Emperor asks: a person whose chi pulse is very rapid and whose sinews are tense and stand out — what illness is this? Qi Bo answers: this is what is called sinew enduring illness. Such a person’s abdomen is necessarily tense; if white or black colour shows, the illness is severe.
Dì yuē: rén yǒu bìng tóu tòng, yǐ shuò suì bù yǐ, cǐ ān dé zhī? Míng wéi hé bìng? Qí bó yuē: dāng yǒu suǒ fàn dà hán, nèi zhì gǔ suǐ, suǐ zhě yǐ nǎo wéi zhǔ, nǎo nì gù lìng tóu tòng, chǐ yì tòng, bìng míng yuē jué nì. Dì yuē: shàn.
The Emperor asks: a person with head pain that has not let up for several years — how was this got, and what is it named? Qi Bo answers: he must have offended against great cold, which reached inward to the bone marrow. The marrow takes the brain as its master; when the brain counterflows it causes head pain, and the teeth ache as well. The illness is named counterflow inversion. The Emperor says: good.
Dì yuē: yǒu bìng kǒu gān zhě, bìng míng wéi hé? Hé yǐ dé zhī? Qí bó yuē: cǐ wǔ qì zhī yì yě, míng yuē pí dān.
The Emperor asks: a person with a sweet taste in the mouth — what is the illness named, and how was it got? Qi Bo answers: this is an overflow of the five qi. It is named spleen dan.
Fú wǔ wèi rù kǒu, cáng yú wèi, pí wéi zhī xíng qí jīng qì, jīn yè zài pí, gù lìng rén kǒu gān yě. Cǐ féi měi zhī suǒ fā yě, cǐ rén bì shuò shí gān měi ér duō féi yě.
Now the five flavours enter the mouth and are stored in the stomach, and the spleen moves their essential qi for it; the fluids lodge in the spleen, and so the mouth is made sweet. This is brought on by rich and fine food: such a person must often eat sweet and fine things, and much fat.
Féi zhě lìng rén nèi rè, gān zhě lìng rén zhōng mǎn, gù qí qì shàng yì, zhuǎn wéi xiāo kě. Zhì zhī yǐ lán, chú chén qì yě.
Fat makes a person hot within; sweetness makes the centre full. Therefore the qi overflows upward and turns into wasting and thirsting. Treat it with lan, to remove the stale qi.
Dì yuē: yǒu bìng kǒu kǔ, qǔ yáng líng quán, kǒu kǔ zhě, bìng míng wéi hé? Hé yǐ dé zhī? Qí bó yuē: bìng míng yuē dǎn dān.
The Emperor asks: a person with a bitter taste in the mouth, for whom one takes Yang Mound Spring — what is the bitter mouth named, and how was it got? Qi Bo answers: the illness is named gallbladder dan.
Fú gān zhě, zhōng zhī jiàng yě, qǔ jué yú dǎn, yān wéi zhī shǐ. Cǐ rén zhě, shuò móu lǜ bù jué, gù dǎn xū qì shàng yì, ér kǒu wéi zhī kǔ.
Now the liver is the general within; decision is taken from the gallbladder, and the throat serves as its envoy. Such a person plans and ponders often without deciding; therefore the gallbladder qi is empty and overflows upward, and the mouth is bitter because of it.
Zhì zhī yǐ dǎn mù shù, zhì zài yīn yáng shí èr guān xiāng shǐ zhōng.
Treat it by means of the gallbladder’s alarm point and transport point; the treatment is set out in the Yin Yang Twelve Officials Serving One Another.
Dì yuē: yǒu lóng zhě, yī rì shuò shí sōu, cǐ bù zú yě. Shēn rè rú tàn, jǐng yīng rú gé, rén yíng zào shèng, chuǎn xī qì nì, cǐ yǒu yú yě.
The Emperor asks: a person with urinary block who urinates some tens of times a day — this is insufficiency. The body is hot as charcoal, the neck and chest as if barricaded, the ren ying pulse agitated and full, breathing panting and qi counterflowing — this is excess.
Tài yīn mài wēi xì rú fà zhě, cǐ bù zú yě. Qí bìng ān zài? Míng wéi hé bìng? Qí bó yuē: bìng zài tài yīn, qí shèng zài wèi, pō zài fèi, bìng míng yuē jué, sǐ bù zhì.
The greater yin pulse faint and fine as a hair — this too is insufficiency. Where does the illness lie, and what is it named? Qi Bo answers: the illness is in greater yin; its excess is in the stomach, and somewhat in the lung. The illness is named inversion: it is death, and cannot be treated.
Cǐ suǒ wèi dé wǔ yǒu yú, èr bù zú yě. Dì yuē: hé wèi wǔ yǒu yú, èr bù zú? Qí bó yuē: suǒ wèi wǔ yǒu yú zhě, wǔ bìng zhī qì yǒu yú yě; èr bù zú zhě, yì bìng qì zhī bù zú yě.
This is what is called obtaining five excesses and two insufficiencies. The Emperor asks: what are the five excesses and the two insufficiencies? Qi Bo answers: the five excesses are the qi of five disease signs in excess; the two insufficiencies are likewise the insufficiency of the qi of the illness.
Jīn wài dé wǔ yǒu yú, nèi dé èr bù zú, cǐ qí shēn bù biǎo bù lǐ, yì zhèng sǐ míng yǐ.
Now outwardly there are five excesses and inwardly two insufficiencies. This body is neither surface nor interior; that it is a certain death is plain.
Dì yuē: rén shēng ér yǒu bìng diān jí zhě, bìng míng yuē hé? Ān suǒ dé zhī? Qí bó yuē: bìng míng wéi tāi bìng. Cǐ dé zhī zài mǔ fù zhōng shí, qí mǔ yǒu suǒ dà jīng, qì shàng ér bù xià, jīng qì bìng jū, gù lìng zǐ fā wéi diān jí yě.
The Emperor asks: a person born already ill with peak illness — what is the illness named, and how was it got? Qi Bo answers: the illness is named womb illness. It was got while the child was in the mother’s belly: the mother had some great fright, so that the qi rose and did not descend and the essence qi stayed gathered together; thus the child is made to develop peak illness.
Dì yuē: yǒu bìng máng rán, rú yǒu shuǐ zhuàng, qiè qí mài dà jǐn, shēn wú tòng zhě, xíng bù shòu, bù néng shí, shí shǎo, míng wéi hé bìng? Qí bó yuē: bìng shēng zài shèn, míng wéi shèn fēng.
The Emperor asks: a person ill with a swollen, puffy look, as though there were water, whose pulse when felt is large and tense, whose body has no pain and whose frame is not thin, yet who cannot eat and eats little — what is this named? Qi Bo answers: the illness arises in the kidney and is named kidney wind.
Shèn fēng ér bù néng shí, shàn jīng, jīng yǐ, xīn qì wěi zhě sǐ. Dì yuē: shàn.
Where kidney wind is accompanied by inability to eat and by frequent fright, and once the fright has passed the heart qi is wilted, the person dies. The Emperor says: good.
