少 simply means not much — 《說文》 不多也 — and 氣 began as fodder given to a guest before it meant vapour. There is nothing clinical in either graph; the term is the plainest thing a patient says about themselves.
Shuowen

In brief

In chapter 22 it comes with what may be the chapter’s most accurate clinical phrase: too little qi to carry on the breath. The lung has run short, not in air but in the power to continue it, and the sentence catches something every clinician recognises — the pause before the next breath is where it fails, not the breath itself. Deafness and a dry throat arrive in the same sentence, which places the shortage squarely at the lung and nowhere else.

In lung illness there is panting and coughing with counterflowing qi, pain in the shoulder and back, sweating, and pain all through the sacrum and the private parts, the thigh, knee, hip and calf, the shin and the foot; in depletion the qi is too scant to carry on the breath, and there is deafness and dryness of the throat. Take the blood of its channels: the greater yin, the outer side of the foot greater yang, and the inner side of the reverting yin.

In a year of fire in surplus, blazing summer heat spreads and moves and the metal of the lung receives evil. The people are ill with malaria, scantness of qi, coughing and panting, blood escaping upward, blood discharged downward, pouring below, dryness of the throat and deafness, heat in the centre and heat in the shoulder and back; it answers above to Mars. If it is severe there is pain in the chest, propping fullness and pain in the flanks, pain between the breast, the back and the shoulder blades, pain on the inner side of both arms, heat of the body and pain in the bones, and the soaking spread of the illness. The qi of gathering does not move and the qi of growth shines alone; rain, water, frost and cold arrive; it answers above to Mercury. If lesser yin or lesser yang presides above, fire blazes and scorches, the icy springs dry up and things are burnt and withered; the illness turns to delirious speech and raving, coughing and panting with a noisy breath, and, below, severe bleeding and unending discharge. Where Taiyuan is cut off there is death and no cure; it answers above to Mars.
