鬲
Literal
鬲 a three-legged cauldron (《說文》: 鼎屬 — a kind of tripod vessel); borrowed for the partition that divides the trunk, the way the vessel's legs divide its fire. Read gé; the later character 膈 is the same word with the flesh radical added.
Shuowen
《說文解字》:鬲,鼎屬也(段注:引申為膈膜之膈;「膈」為後起字,从肉)。

In brief

The chapter’s oddest penalty. Striking the diaphragm does not kill quickly — 不過一歲必死, the person will die within a year — and yet it is listed with the strikes that kill in a day. It appears twice: once among the fatal strikes, and once in the rule for avoiding them, where the diaphragm’s place is named as one of the places you may needle. It is the one structure in the chapter that is both lethal and passable.

Whenever one needles the chest or abdomen, one must avoid the five depots. Striking the heart means death within one circuit. Striking the spleen means death in five days. Striking the kidney means death in seven days. Striking the lung means death in five days. Striking the diaphragm is always a wound to the interior: even if the illness appears to mend, the person will certainly die within a year. Needling so as to avoid the five depots is knowing rebellion and compliance. What is called compliance is the place of the diaphragm and of the spleen and kidney; one who does not know it does the opposite.

How it has been translated

Every rendering below is a decision about which reader is being addressed. There is no neutral option.

Common Englishthe diaphragmThe anatomical term, and correct; there is nothing to decide.
This sitethe diaphragmKept. What deserves a note is the character: 鬲 is a cauldron that stands on legs over the fire, and the borrowed sense is of a partition inside the trunk, for which the later 膈 adds the flesh radical. What the chapter calls 鬲 separates the chest from the abdomen — which is why striking it is not instantly mortal and yet, in the chapter's judgement, never survivable.
