The Master asked: “The pulse has yin and yang — what does that mean?” The answer: “When the pulse is large, floating, rapid, moving or slippery, it is named yang; when it is deep, astringent, weak, wiry or faint, it is named yin. In yin disease, seeing a yang pulse, the patient lives; in yang disease, seeing a yin pulse, the patient dies.”

When the inch mouth pulse is floating and large: the floating is vacuity, the large is repletion. At the cubit it makes a closing, at the inch a barrier. Closing, no urine passes; barring, there is vomiting.

Question: the signs resemble yangdan; treating by the rule only made it worse — cold limbs, a dry throat, cramped calves and delirious speech. The master said: by midnight the hands and feet will be warm and the legs will straighten. It turned out as he said. How could he know? Answer: the cunkou pulse is floating and large; floating means wind, large means emptiness. Wind engenders faint heat, emptiness cramps the calves. The signs resemble the Cinnamon Twig pattern, but aconite was added in between and cinnamon increased to make sweat come out; aconite warms the channels, so the yang was lost. Cold limbs, dry throat, restlessness, yangming binding within, delirious and confused speech: again drink Liquorice and Dried Ginger Decoction. At midnight the yang returns, both feet grow hot, the calves are still slightly cramped; give Peony and Liquorice Decoction again and the calves straighten; with the Qi-Settling Decoction a slight looseness stops the delirium. Thus one knows the illness can be cured.

After Cinnamon Twig Decoction, with heavy sweating and a surging, large pulse, give Cinnamon Twig again as before. If the pattern is malaria-like with two attacks a day, sweat will surely resolve it: Cinnamon Twig Two and Ephedra One Decoction is fitting.

Asked: “In pulse illness, how does one tell whether recovery has come?” The answer: “When at cunkou, the bar and the cubit the three positions are equal in large and small, floating and deep, slow and rapid, then even if cold and fever are not yet resolved, the yin and yang of the pulse are at peace, and however severe the illness, it will recover.”
