Distinguishing the Pulse

Biàn mài fǎ dì yī

Questions and answers on the pulse, in fifty-one passages. The chapter opens by dividing the pulses into yin and yang: large, floating, rapid, stirring and slippery are yang; deep, rough, weak, string-like and faint are yin. From there it takes up the single pulses one by one — slow, moderate, flooding, tense, hollow, wiry, firm, hidden, weak, scattered, thready, deep, slippery, stirring, intermittent — names them, and tells what each is like under the finger. It then turns to the mistakes a taker can make: a pulse read before the patient has settled, a pulse taken through clothing, a pulse confused by a full stomach or an empty one. The last passages describe the pulse after a long illness, when the body has not yet returned to itself.Cheng Wuyi's commentary runs beneath the classic in small graphs throughout, glossing each name and drawing the whole into a single argument: that the pulse is read from yin and yang, and that yin and yang are read from the five phases. It is quoted selectively here, marked 成注(節).

The Master asked: “The pulse has yin and yang — what does that mean?” The answer: “When the pulse is large, floating, rapid, moving or slippery, it is named yang; when it is deep, astringent, weak, wiry or faint, it is named yin. In yin disease, seeing a yang pulse, the patient lives; in yang disease, seeing a yin pulse, the patient dies.”

Asked: “When the pulse has yang binding and yin binding, how are they told apart?” The answer: “If the pulse is floating and rapid, with ability to eat but no stool, this is excess: it is called yang binding, and it is expected to worsen on the seventeenth day. If the pulse is deep and slow, with no appetite, heaviness of body and a hard stool instead, it is called yin binding, and it is expected to worsen on the fourteenth day.”

If a rapid pulse appears at the bar, with no head or tail above or below, the size of a bean, stirring and shaking, it is the qi of yin and yang striking each other, and is therefore called moving. When the yang pulse is floating, large and soft, and the yin pulse is floating, large and soft, the yin and yang pulses being equal, it is called relaxed.

A pulse wiry and large: wiry means diminished, large means hollow, diminished means cold, hollow means vacuous; cold and vacuity striking together is called the leather pulse. In women it brings miscarriage and dripping menses, in men the loss of blood and semen.

Asked: “Some patients shiver and then sweat, and thereby recover — why?” The answer: “The pulse is floating and tight, yet hollow on pressure: this is root vacuity, so there is shivering and then sweating. The person was vacuous to begin with, hence the shivering; because the pulse is floating, sweating brings resolution.”

If the pulse is floating and rapid but not hollow on pressure, the person is not vacuous; if recovery comes of itself, there is only sweating, without shivering.

Asked: “Some recover by sweating without shivering — why?” The answer: “The pulse is large, floating and rapid; hence one knows there will be no shivering, only sweating, and recovery.”

Asked: “Some recover without shivering and without sweating — why?” The answer: “The pulse is of itself faint, because the patient has already been made to sweat, or has vomited, been purged, or lost blood, so that no fluid is left within. Yin and yang then harmonise of themselves and recovery must follow; hence no shivering and no sweating.”

Asked: “On the third day of cold damage the pulse is floating, rapid and faint, and the patient’s body is cool and settled — why?” The answer: “This is about to resolve, and it will resolve at midnight. Resolving with a floating pulse means a sudden sweat; resolving with a rapid pulse means the appetite will return; resolving with a faint pulse means a great sweat.”

Asked: “In pulse illness, how does one tell whether recovery has come?” The answer: “When at cunkou, the bar and the cubit the three positions are equal in large and small, floating and deep, slow and rapid, then even if cold and fever are not yet resolved, the yin and yang of the pulse are at peace, and however severe the illness, it will recover.”

At the beginning of summer a flooding, large pulse is proper to the season. If the patient suffers bodily pain and heaviness, sweating must be induced; if the next day there is no pain or heaviness, no sweating is needed; if sweat comes of itself, dribbling, recovery follows the next day. Why so? At the beginning of summer a flooding, large pulse is the pulse of the season, and so it comes about. The four seasons are judged by the same rule.

Falling ill at midday, one is attacked in the yang; falling ill at midnight, one is attacked in the yin. When yang is disharmonious it is harmonised by yin, so resolution comes at midnight; when yin is disharmonious it is harmonised by yang, so resolution comes at midday. The classic says: “Use yang to harmonise yin, use yin to harmonise yang.”

When the fuyang pulse is slow and relaxed, the stomach qi is as usual. When it is floating and rapid, floating injures the stomach and rapid stirs the spleen; this is not the original disease but the work of a physician who purged wrongly. Ying and wei sink inward, the rapid quality fades first, the pulse becomes merely floating, and the patient must have a hard stool and belching that relieves him. Why? Originally the rapid pulse stirred the spleen; the rapid fades first, so one knows the spleen qi is unregulated, the stool hard, and belching relieves. Now the pulse is floating instead and the rapid has changed to faint; the evil qi remains alone, there is hunger in the chest, the evil heat fails to digest grain, and there is tidal fever with thirst. The rapid pulse should turn slow and relaxed; if the pulse’s measures before and after follow the rule, the patient will be hungry; if the rapid pulse does not keep time, evil sores will form.

The master said: “When a patient’s pulse is faint and astringent, he has been harmed by the physician. Sweating him greatly and purging him repeatedly, the man loses blood. The illness should bring aversion to cold and afterwards fever, without cease.” In summer’s great heat he wants thick garments; in winter’s great cold he wants to go bare. The reason is that faint yang means aversion to cold and weak yin means fever. The physician sweated him and made the yang qi faint, then purged him greatly and made the yin qi weak. In the fifth month the yang qi is in the exterior and the stomach is cold within; because the yang qi is faint inside it cannot master the cold, so he wants thick garments. In the eleventh month the yang qi is in the interior and the stomach is hot and restless; because the yin qi is weak inside it cannot master the heat, so he wants to go bare. Moreover the yin pulse is slow and astringent, so one knows the blood is lost.

When the cunkou pulse is floating and large and the physician nonetheless purges, this is a gross error. Floating means no blood, large means cold; cold qi striking together gives borborygmus. The physician, not knowing, gives cold water to drink and makes him sweat greatly; water meets the cold qi, the colds must strike together, and the man is seized with choking.

When the fuyang pulse is floating, floating means vacuity; floating and vacuity striking together produce choking of qi, which is to say the stomach qi is vacuous and exhausted. A slippery pulse means hiccough; this is the physician’s fault, exacting from vacuity and taking from excess, guarding emptiness and driving the blood. If the pulse is floating and the nose dry, there will certainly be nosebleed.

When the cunkou pulse is tight in both yin and yang, the rule is that clear evils strike the upper burner and turbid evils the lower burner. Clear evils striking above are called purity; turbid evils striking below are called turbidity. When yin is struck by evil there must be inner trembling; the exterior qi is slightly vacuous and the interior qi does not guard, so the evil is let into the yin. When yang is struck by evil there must be fever, headache, stiff neck, cramped nape, low back pain and sore shins; this is yang struck by mist and dew, hence the saying that clear evils strike above and turbid evils below. Yin qi makes one tremble; the feet and knees are cold against the flow, urine and stool pass at random; the exterior qi is slightly vacuous, the interior qi slightly tense, the three burners are muddled, inside and outside do not communicate, the upper burner is blocked, the zang qi is smoked, the mouth rots and the gums are eaten. When the middle burner is ungoverned, stomach qi rushes up, spleen qi does not turn, the stomach becomes turbid, ying and wei do not communicate, and blood congeals and does not flow. If the wei qi is the first to pass through, the urine is red-yellow; striking with heat and employed by heat, it roams the channels and network, entering and leaving the zang and fu, and wherever the heat qi passes, abscesses and pus form. If the yin qi is the first to pass through, the yang qi is reversely faint and yin has nothing to employ; guest qi enters within, sneezing expels it, the voice is choked and the throat blocked; cold and reversal pursue each other and are embraced by heat, blood congeals and descends of itself like pig’s liver; yin and yang both reverse, spleen qi is orphaned and weak, the five fluids pour down, the lower burner does not close, the clear stool drops heavily, making the stool frequent and difficult, and pressing pain knots about the navel — life will be hard to keep whole.

If the pulse is harmonious, then even with great restlessness, heaviness of the eyelids and yellow at the inner margin of the face, it is about to resolve.

A floating, rapid pulse: floating means wind, rapid means vacuity; wind means heat, vacuity means cold; wind and vacuity striking together produce sprinkling shivering aversion to cold.

A floating, slippery pulse: floating is yang, slippery is excess; yang and excess striking together make the pulse rapid and swift, and the wei qi loses its measure. A floating, slippery pulse that is rapid and swift, with fever and sweating, is untreatable.

In cold damage with coughing, counterflow and rising qi, a scattering pulse means death, for the form is damaged.

Textual notes

Readings follow the base edition; significant variants are recorded here.
