The Normal Pulse

Píng mài fǎ dì èr

The second chapter on the pulse, in seventy-one passages, and the one that teaches what a normal pulse is before it teaches what a sick one is.It begins with a set piece: the three regions, yin and yang pressing on one another, the blood and qi of the constructive and defensive, moving in and out with the breath, moistening and spreading. Spring is string-like, autumn floating, winter deep — the pulse takes its shape from the season. From this measure it argues that a pulse is called excessive or deficient only against a standard, and that the standard is the man himself: his age, his build, his work, the hour of the day.The later passages turn to the pulse at the inch, the bar and the cubit, to the five viscera each appearing at its own position, and to the signs by which a physician knows that what he feels is not the pulse of this man but the pulse of a season, or of a course of treatment, or of a dying body.Cheng Wuyi's commentary is quoted selectively, marked 成注(節).

Asked: “The pulse has three parts; yin and yang encroach on one another; the ying and wei, the blood and the qi, are within the body. Breath goes out and in, rises and descends within it; by the breath they are spread abroad and the fluids flow freely. They move with the seasons, and their form is imaged in the pulse: in spring it is wiry, in autumn floating, in winter deep, in summer flooding. Looking at the complexion and feeling the pulse, the sizes differ. Within a single breath the changes have no constant measure; the inch and the cubit are uneven, now short, now long; above and below go contrary, now present, now gone. The illness shifts with them, and the pulse rises and falls. My mind is confused and I lose the guiding rules — I beg you to set it all out plainly.” The Master said: “What you ask is the very root of the healing art. The pulse has three parts: the cubit, the inch and the gate. The inch is the upper part, the gate the middle, the cubit the lower. The ying and wei move through them without losing their balance.”

The kidney pulse is deep, the heart pulse flooding, the lung pulse floating, the liver pulse wiry. These are their own constant measures, not off by a grain’s weight.

The qi of the pulse goes out and in, rises and descends, turning like the water of the clepsydra. When the clepsydra has run two marks, the pulse has made one circuit, and so it goes round without end.

When the pulse changes and one phase encroaches on another, yin and yang offend each other. With wind the pulse is floating and vacuous; with cold it is firm and hard. Where water is stored within, the pulse is deep and hidden; where there is propulsive rheum, it is tense and wiry. A moving pulse gives pain; a rapid pulse gives fever and restlessness.

When the pulse does not answer the illness, know that transmission has brought it about. The three parts each have their own domain, and so each illness shows a different face.

Excess is strange, and so is deficiency. Evil never appears for nothing; there must be some mischief within. Examine whether it is in the exterior or the interior, and the three burners are told apart; know where it has settled, then weigh and observe, measure the organs, and your seeing will be as of a spirit. I record it for you in order, to pass on to worthy men.

Asked: “The best physician knows by looking, the middling by asking, the lesser by feeling the pulse; I would hear the reason.” The Master said: “When the family calls and says the patient has fever and bodily pain, yet the patient lies still at ease — the physician arrives, feels a deep and slow pulse, and knows the illness is passing. How does he know? When the exterior is diseased the pulse should be floating and large; now it is deep and slow instead, and so he knows it will heal.”

But if, lying facing the wall, he does not start up to look when he hears the physician come, answers in halting words, and swallows his spittle while the pulse is felt — this is feigned illness. The Master says: ‘When I came before I did not see these signs; now they have changed strangely — this is called calamitous strangeness.’ Again he is asked: ‘By what cause does this vomiting and diarrhoea arise?’ The answer: ‘Perhaps medicine taken at an earlier time is only now breaking out; that is why it is called calamitous strangeness.’”

Suppose the pulse comes faint and leaves large: this is called the reversed, and the disease is within. Suppose it comes with a small head and a large root: this is called the overturned, and the disease is in the exterior. When the upper is faint and the head small, there will be sweating; when the lower is faint and the root large, there is gate-and-barrier blockage, and no urine passes. If the head has no sweat, it can be treated; if it sweats, death.

The lung pulse of the west — what is its form? The Master said: “The lung is metal, named great yin; its pulse is like a hair, floating. When lung disease of itself gets this pulse, or gets the relaxed and slow, it will heal in either case; if it gets the rapid, it worsens. How is that known? The rapid is the fire of the south, and fire overcomes the metal of the west; by rule there should be swelling and abscess, which is hard to treat.”

Asked: “Getting a hairy, floating pulse in the second month, why pronounce that death will come in autumn?” The Master said: “In the second month the pulse should be soft and weak; getting the hairy and floating instead, one knows death will come in autumn. In the second month the liver has charge; the liver pulse belongs to wood and should be soft and weak. Getting the hairy and floating instead, this is the lung pulse. The lung belongs to metal, and metal comes to overcome wood; so death in autumn is known. All other cases follow this pattern.”

When the person is diseased but the pulse is not, it is called inner vacuity, for there is no grain spirit; though he is distressed, he suffers no harm.”

cùn kǒu mài wēi ér sè wēi zhě wèi qì shuāi sè zhě róng qì bù zú wèi qì shuāi miàn sè huáng róng qì bù zú miàn sè qīng róng wéi gēn wèi wéi yè róng wèi jù wēi zé gēn yè kū gǎo ér hán lì kài nì tuò xīng tù xián mò yě
When the inch mouth pulse is faint and astringent: the faint means the wei qi declines, the astringent means the ying qi is insufficient. When the wei qi declines the face is yellow; when the ying qi is insufficient the face is green. The ying is the root, the wei the leaf; when both are faint the root and leaf wither, and there is shivering with cold, coughing with rising qi, spitting of a rank taste, and vomiting of thin fluid.
fū yáng mài fú ér kōu fú zhě wèi qì shuāi kōu zhě róng qì shāng qí shēn tǐ shòu jī ròu jiǎ cuò fú kōu xiāng bó zōng qì shuāi wēi sì shǔ duàn jué
When the instep yang pulse is floating and hollow: the floating means the wei qi declines, the hollow means the ying qi is injured; the body grows thin and the flesh is scaled like armour. Floating and hollow striking together, the ancestral qi wanes, and the four dependencies of skin, flesh, fat and marrow are cut off from nourishment.
cùn kǒu mài wēi ér huǎn wēi zhě wèi qì shū shū zé qí fū kōng huǎn zhě wèi qì shí shí zé gǔ xiāo ér shuǐ huà yě gǔ rù yú wèi mài dào nǎi xíng ér rù yú jīng qí xuè nǎi chéng róng shèng zé qí fū bì shū sān jiāo jué jīng míng yuē xuè bēng
When the inch mouth pulse is faint and slack: the faint means the wei qi leaks, and leaking leaves the skin empty; the slack means the stomach qi is replete, and repletion digests grain and transforms water. When grain enters the stomach the pulse tract then moves; when it enters the channels the blood is then formed. If the ying is abundant the skin must leak, the three burners lose their constant course, and it is named blood collapse.
fū yáng mài wēi ér jǐn jǐn wéi hán wēi zé wéi xū wēi jǐn xiāng bó zé wéi duǎn qì
When the instep yang pulse is faint and tight: the tight is cold, the faint is vacuity; faint and tight striking together gives shortness of breath.
shào yīn mài ruò ér sè ruò zhě wēi fán sè zhě jué nì
When the lesser yin pulse is weak and astringent: the weak gives slight restlessness, the astringent gives reversal of the limbs.
fū yáng mài bù chū pí bù shàng xià shēn lěng fū yìng
When the instep yang pulse does not come forth, the spleen cannot move up and down; the body is cold and the skin hard.
shào yīn mài bù zhì shèn qì wēi shǎo jīng xuè bēn qì cù pò shàng rù xiōng gé zōng qì fǎn jù xuè jié xīn xià yáng qì tuì xià rè guī yīn gǔ yǔ yīn xiāng dòng lìng shēn bù rén cǐ wéi shī jué dāng cì qī mén jù què
When the lesser yin pulse does not arrive, the kidney qi is faint, essence and blood are scanty, and rushing qi presses urgently upward into the chest and diaphragm; the ancestral qi gathers instead, blood knots below the heart, the yang qi retreats downward, heat returns to the inner thighs and stirs with the yin, so that the body is without feeling. This is corpse-like reversal; one should needle qimen and juque.
cùn kǒu mài wēi chǐ mài jǐn qí rén xū sǔn duō hàn zhī yīn cháng zài jué bù jiàn yáng yě
When the inch mouth pulse is faint and the cubit pulse tight, the person is depleted and sweats much; one knows the yin is always present and the yang is utterly unseen.
cùn kǒu zhū wēi wáng yáng zhū rú wáng xuè zhū ruò fā rè zhū jǐn wéi hán zhū chéng hán zhě zé wéi jué yù mào bù rén yǐ wèi wú gǔ qì pí sè bù tōng kǒu jí bù néng yán zhàn ér lì yě
At the inch mouth, all faint pulses mean yang loss, all soft pulses mean blood loss, all weak pulses mean fever, all tight pulses mean cold. Whenever cold rides in on vacuity there is reversal, with clouded dizziness and no feeling, because the stomach has no grain qi and the spleen is astringent and blocked; so the mouth is stiff and speech fails, and there is shivering and cold trembling.
wèn yuē rú ruò hé yǐ fǎn shì shí yī tóu shī yuē wǔ zàng liù fǔ xiāng chéng gù lìng shí yī
Asked: “Why does the soft and weak pulse come to eleven heads?” The Master said: “The five stores and six palaces encroach on one another, and so it makes eleven.”
wèn yuē hé yǐ zhī chéng fǔ hé yǐ zhī chéng zàng shī yuē zhū yáng fú shuò wéi chéng fǔ zhū yīn chí sè wéi chéng zàng yě
Asked: “How does one know it encroaches on a palace, and how that it encroaches on a store?” The Master said: “All yang pulses that are floating and rapid encroach on the palaces; all yin pulses that are slow and astringent encroach on the stores.”

Textual notes

Readings follow the base edition; significant variants are recorded here.
