A Discourse on Root and Branch and the Transmission of Disease

Biāo běn bìng chuán lùn

Suwen 65 is the chapter that gives the whole book two of its most durable words. 標 is the branch and 本 is the root, and everything else here is an application of that pair: which of the two is treated first, and why. It falls into three movements. The first is the principle, and it comes as a quotation inside 歧伯's answer. An illness may sit in the branch and be sought in the branch, sit in the root and be sought in the root, or — and this is the point — sit in one and be sought in the other; and the same is true of the treatments, which take the branch or the root, go against the illness or go with it, and succeed either way. From this comes the maxim the chapter is remembered for: knowing counter and accord, one proceeds correctly and has nothing to ask about; knowing root and branch, one is fitting ten thousand times out of ten thousand; not knowing them, one is simply practising at random. The knowledge is small to hold and large in reach — from the shallow one knows the deep, from what is near one knows what is far, and speaking of one thing one knows a hundred. The second movement is the working rule, and it is the part physicians have actually used. It is a list of paired cases. Illness before counter-flow, counter-flow before illness, cold before illness, illness before cold, heat before illness, illness before discharge, discharge before another illness: all of these take the root. Two take the branch, and both of them are fullness in the centre, because a centre that will pass nothing can receive nothing either. Then the rule for the passages: when the lesser and the greater do not flow freely, treat the branch; when they flow, treat the root. Then the rule for surplus and insufficiency: with surplus, go from the root outward to the branch; with insufficiency, from the branch inward to the root. Then the rule for severity: in the light cases branch and root are treated side by side, in the severe ones a single course is taken alone. The third movement is the catalogue that gives the chapter its second name. Seven illnesses — heart, lung, liver, spleen, kidney, stomach, bladder — each with the day on which each symptom arrives, the day on which the person dies if it has not abated, and the hour at which death comes, given twice over, once for winter and once for summer. The hours are the old names for the divisions of the day: 夜半 at midnight, 日中 at midday, 日入 at sunset, 日出 at sunrise, 早食 at the early meal, 晏食 at the late meal, 人定 when people have settled for the night, 大晨 at the great dawn, 晏晡 at the late afternoon meal, 日昳 as the sun begins to decline, 雞鳴 at cockcrow, 下晡 as the afternoon falls. 張介賓 notes that the intervals are not uniform and that the text is not to be forced into an arithmetic; it is given here as it stands. The chapter closes on the rule that follows from the catalogue. Illnesses that pass from viscus to viscus in this order all have their date and are not to be punctured; where the transmission passes over a viscus and stops, and where it goes on to a third or fourth viscus, puncture may be applied.

Huáng dì wèn yuē: bìng yǒu biāo běn, cì yǒu nì cóng, nài hé?
Huang Di asked: illness has a root and a branch, and puncture has what runs counter and what accords. How is this?
Qí bó duì yuē: fán cì zhī fāng, bì bié yīn yáng, qián hòu xiāng yìng, nì cóng dé shī, biāo běn xiāng yí.
Qi Bo replied: the rule for every puncture is that one must separate yin from yang, let what comes before and what follows answer one another, let what runs counter and what accords each be applied where it belongs, and let root and branch change places with one another.
Gù yuē: yǒu qí zài biāo ér qiú zhī yú biāo, yǒu qí zài běn ér qiú zhī yú běn, yǒu qí zài běn ér qiú zhī yú biāo, yǒu qí zài biāo ér qiú zhī yú běn.
Hence it is said: some illnesses sit in the branch and are sought in the branch; some sit in the root and are sought in the root; some sit in the root yet are sought in the branch; and some sit in the branch yet are sought in the root.
Gù zhì yǒu qǔ biāo ér dé zhě, yǒu qǔ běn ér dé zhě, yǒu nì qǔ ér dé zhě, yǒu cóng qǔ ér dé zhě.
Thus among treatments there are those that succeed by taking the branch, those that succeed by taking the root, those that succeed by taking against it, and those that succeed by taking in accord with it.
Gù zhī nì yǔ cóng, zhèng xíng wú wèn, zhī biāo běn zhě, wàn jǔ wàn dàng. Bù zhī biāo běn, shì wèi wàng xíng.
Therefore, knowing what runs counter and what accords, one proceeds correctly and has nothing to ask about; one who knows root and branch is fitting in ten thousand acts out of ten thousand. Not to know root and branch is called reckless proceeding.
Fú yīn yáng nì cóng biāo běn zhī wéi dào yě, xiǎo ér dà, yán yī ér zhī bǎi bìng zhī hài; shǎo ér duō, qiǎn ér bó, kě yǐ yán yī ér zhī bǎi yě.
Now yin and yang, counter and accord, root and branch, as a way: from the small one reaches the great, naming one thing and knowing the harm in a hundred illnesses; from the few one reaches the many, from the shallow the broad, so that one may speak of one and know a hundred.
Yǐ qiǎn ér zhī shēn, chá jìn ér zhī yuǎn, yán biāo yǔ běn, yì ér wù jí. Zhì fǎn wéi nì, zhì dé wéi cóng.
From the shallow know the deep; examining what is near, know what is far. To speak of branch and root is easy, yet one may still fall short of it. To treat against the illness is to go counter; to treat in keeping with it is to accord.

When illness comes first and counter-flow of qi follows, treat the root; when counter-flow comes first and illness follows, treat the root; when cold comes first and illness arises from it, treat the root; when illness comes first and cold arises from it, treat the root; when heat comes first and illness arises from it, treat the root; when heat comes first and fullness in the centre arises from it, treat the branch; when illness comes first and discharge follows, treat the root; when discharge comes first and another illness arises from it, treat the root.

One must first bring this into order, and only then treat the other illness. When illness comes first and fullness in the centre follows, treat the branch; when fullness in the centre comes first and vexation of the heart follows, treat the root.
Rén yǒu kè qì, yǒu tóng qì. Xiǎo dà bù lì zhì qí biāo, xiǎo dà lì zhì qí běn.
A person has visiting qi and has kindred qi. When the lesser and the greater do not flow freely, treat the branch; when the lesser and the greater do flow freely, treat the root.
Bìng fā ér yǒu yú, běn ér biāo zhī, xiān zhì qí běn, hòu zhì qí biāo; bìng fā ér bù zú, biāo ér běn zhī, xiān zhì qí biāo, hòu zhì qí běn.
When the illness erupts and there is surplus, go from the root to the branch: treat the root first and the branch afterwards. When the illness erupts and there is insufficiency, go from the branch to the root: treat the branch first and the root afterwards.
Jǐn chá jiàn shèn, yǐ yì tiáo zhī, jiàn zhě bìng xíng, shèn zhě dú xíng. Xiān xiǎo dà bù lì ér hòu shēng bìng zhě, zhì qí běn.
Examine carefully the lightness and the severity, and regulate it by judgement: in the light cases the two proceed side by side, in the severe ones a single course is taken alone. When blockage of the lesser and the greater comes first and illness arises afterwards, treat the root.

Textual notes

Readings follow the base edition; significant variants are recorded here.
