胃 wèi, the stomach; 脘 wǎn, the duct or cavity of the stomach — the part that receives what is swallowed. In the chapter it is named as the reason food will not go down: the stomach duct is blocked. It comes at the end of a run of explanations that all place the origin of wind water's signs in the stomach — the rumbling, the heaviness that makes walking hard, the fretting and inability to eat.

In brief

The receiving part of the stomach. The chapter names it once, as the reason food will not go down in wind water — the duct is blocked. It belongs to a small run of explanations that place the disease’s origin in the stomach: the rumbling in the belly, the heaviness that makes walking hard, and the fretting with inability to eat.

How it has been translated

Every rendering below is a decision about which reader is being addressed. There is no neutral option.

Wiseman & Fengstomach ductThey render 脘 as duct, which keeps the sense of a passage rather than a chamber.
Unschuldstomach cavityUnschuld takes 脘 as the hollow of the stomach, which matches the older lexicographers.
This sitestomach ductWe keep duct: the chapter's complaint is that food will not go down, and a passage that can be blocked describes that better than a cavity.
