逆从
Literal
逆 to go against, to meet head-on; 從 to follow, to go along with. The pair names the two directions a treatment or a disease can take relative to what is proper, and the physician's first duty is to tell them apart.
Shuowen

In brief

The chapter uses the pair twice, and both times as something the physician is supposed to examine and fails to. First of what the late-age physician does not do: 不審逆從. Then of what he does do: 逆從到行 — he walks the pair upside down, taking what goes against for what follows. Wang Bing’s gloss is severe: to treat by inverted reason is not merely to harm the patient’s spirit; set such a man to advise a ruler and the state will not keep its peace. The opposite skill appears in Suwen 9 as 謹熟陰陽,無與衆謀.

The Emperor says: when an illness first arises it is exceedingly minute and fine, and it must first bind in the skin. Yet now the good practitioners all say that once the disease is formed and named rebellion, the needle and stone cannot treat it and good drugs cannot reach it. These same practitioners have the methods and keep the numbers; they are close to the patient as brothers, near or far; the sound of his voice is daily in their ears and the five colours daily before their eyes — and still the illness is not cured. Can it be that they were not early enough?

Cold at its extreme engenders heat; heat at its extreme engenders cold. Cold qi produces turbidity, hot qi produces clarity.
