Opposing and Following, Fat and Thin

Nì shùn féi shòu

The chapter opens with a question about method and answers it with carpentry. A craftsman does not guess at length or draw circles without a compass; the physician likewise works from measures, not from inspired improvisation. Qi Bo then shows what such a measure looks like in practice: the same needle is used differently on the fat, the thin, the ordinary, the strong and the infant, and the differences are stated as depth, retention and number of points. The middle section turns from body types to the movement of qi and blood, using two images — opening a channel above a deep pool, and clearing a dug course. Both describe treatment as the removal of an obstruction rather than the application of force; the water does the work once the gate is open. The last third is the chapter's most quoted part, a compact account of the direction of the twelve channels, followed by a long description of the thoroughfare vessel, which rises to the nasopharynx, descends to the inner thigh and shin, and sends a branch to the instep. It is here that the book explains why the lesser yin, alone among the yin, appears to run downward — because the thoroughfare vessel runs with it. The chapter ends not with a rule but with admiration. The graded rules are worth setting side by side, because they show a single principle applied five times. The fat and the strong who are heavy are needled deep, long and many; the thin, the brisk and the infant are needled shallow, brief and few. What changes is not the aim but the amount of injury the patient can bear. The ordinary person, being neither, is needled by the regular number — which is to say that the "regular" is a middle term, not a standard. The thoroughfare vessel passage is also the chapter's answer to a puzzle in the channel system. If all yin runs upward to the chest, why does the lesser yin of the foot appear to descend? Qi Bo's reply is that what descends is not the channel but the sea that flows beside it, sending branches to the nasopharynx above and the instep below. It is a good example of how the book solves anatomical objections by redescription rather than correction.

The Yellow Emperor asked Qi Bo: I have heard the way of needling from you, Master, and its many parts are all exhausted. Your way, when applied, seems to leave nothing to hold to, and yet what it rests on has nothing rigid about it. Is your learning ripe from study, or do you examine things and let it arise in the heart?

Qi Bo said: The sage who makes a way joins it above to heaven, below to earth, and in the middle to human affairs; he must have a clear method by which measures are set up, a pattern and a frame, and only then can it be transmitted. So a carpenter cannot let go of his rule and guess at length, nor abandon the inked line and make timber level; a wheelwright cannot put aside the compass and make a circle, nor remove the square and make a right angle. To know how to use these is simply the nature of things, an easy teaching, the constancy of opposing and following.

Qi Bo said: Opening a channel above a deep pool lets the water run dry without effort; following a dug course and clearing the rush lets the channel be opened. This speaks of the slippery and the rough in qi, the clear and the turbid in blood, the opposing and following of their movement.

Qi Bo said: When the years are prime, the frame large, blood and qi full, skin and hide firm, and evil is added — needle such a one deeply and leave the needle. This is the fat person: broad in shoulder, armpit and neck, thin flesh and thick skin, of black colour, with heavy hanging lips, whose blood is black and turbid and whose qi is rough and slow. In his dealings he is greedy in taking and giving. Needle him deeply, leave the needle, and increase the number of points.

The Emperor said: The lesser yin vessel alone runs downward — why?
Qí Bó yuē: bù rán. fū chōng mài zhě, wǔ zàng liù fǔ zhī hǎi yě, wǔ zàng liù fǔ jiē bǐng yān.
Qi Bo said: Not so. The thoroughfare vessel is the sea of the five zang and six fu, and all of them receive from it.

Its upper part emerges at the nasopharynx, seeping into the yang and irrigating the essences.

Its lower part pours into the great luo of the lesser yin, emerges at Qijie, follows the inner edge of the inner thigh, enters the popliteal hollow, runs hidden within the shin bone, descends behind the inner ankle, and divides as it joins.

Its forward part runs hidden, emerging at the instep, follows the instep down, enters between the great toes, seeps into the network of luo and warms the flesh. So when a separate luo is knotted, the instep cannot move; not moving, there is reversal; reversal means cold.

Qi Bo said: Guide it with words, verify it by pressing; if it is not so, it must move — and only then can the opposing and following of its movement be made clear.

The Emperor said: How difficult! The sage’s making of a way is bright as sun and moon and fine as a hair; but for you, Master, who could speak of it?

Textual notes

Readings follow the base edition; significant variants are recorded here.
