Yangming Disease

Biàn yáng míng bìng mài zhèng bìng zhì dì bā

The yangming chapter, a hundred and eight passages, and the chapter of heat that has settled in the stomach. It opens on the one sentence that holds the whole of yangming: the disease of yangming is excess of the stomach household. The three approaches are then named — taiyang-yangming, which is the splenic constraint; true yangming-yangming, which is the excess itself; and shaoyang-yangming, dryness and irritation in the stomach with difficult stool. The external signs are set out plainly: heat of the body, spontaneous sweat, no aversion to cold, and aversion to heat instead. Much of the early chapter turns on how the disease arrives — sweat, purgation, or promoted urine draining the fluids until the stomach dries and the disease turns to belong to yangming. The middle is given to the three qi-ordering formulas and to the art of knowing when to use them. Great Qi-Ordering Decoction for the hard, full, tidal case; Minor Qi-Ordering Decoction where the knot is slighter; Regulate-the-Stomach Qi-Ordering Decoction where there is irritation without fullness. The test by gas passed down, the caution against purging too early, and the distinction between delirious speech and the repeated Zheng sound are all worked through here, together with the passages on fixed staring, on picking at clothes and feeling the bed, and on the six or seven days of no stool that demand haste. Then come the formulas that are not purgative: White Tiger for the three yang diseased together, White Tiger plus Ginseng for thirst, Pig Hoof Decoction for water and heat in the lower burner, Gardenia and Fermented Soy for the troubled heart after a purge, Minor Bupleurum where the flank is hard and the tongue coated white, Evodia Decoction for vomiting on grain, and Hemp Seed Pill for the splenic constraint. The chapter closes with the yellowing of the body: Yinchenhao Decoction, Gardenia and Corktree Bark, Ephedra with Forsythia Root and Adzuki, and the two suppositories of honey and of pig bile for a stool that cannot pass.Cheng Wuyi's commentary is quoted selectively, marked 成注(節).

In yangming disease there was spontaneous sweat to begin with, and the physician induced sweat again; the illness has eased, yet there is slight irritation and a lack of clarity, because the stool must be hard. The fluids are drained and the stomach dry, so the stool is hard. One should ask how many times a day the patient urines: if it was three or four times and is now twice, the stool will soon come. Now that the urine is less frequent, the fluids are returning to the stomach, and so the stool will soon pass.

In yangming disease with a slow pulse, sweating, and no aversion to cold, the body must be heavy, the breath short, the abdomen full and panting, with tidal fever; the exterior is about to resolve and the interior may be attacked. In yangming disease with tidal fever and slightly hard stool, Major Qi-Ordering Decoction may be given; if the stool is not hard, it is not given. If there has been no stool for six or seven days and dry dung is feared, the way to find out is to give a little Minor Qi-Ordering Decoction. If after it enters the abdomen gas passes down, there is dry dung and it may be attacked. If no gas passes, only the first part is hard and the rest will be loose; it must not be attacked, for attacking brings fullness and inability to eat. If the patient wants water, giving it causes retching; if fever comes afterwards, the stool will be hard again and scanty, and Minor Qi-Ordering Decoction harmonises it. When no gas passes, it must by no means be attacked.

Two or three days after falling ill, with a weak pulse, no Taiyang or Bupleurum signs, irritation, and hardness below the heart, by the fourth or fifth day, though the patient can eat, give a little Minor Qi-Ordering Decoction to harmonise gently and give some ease; by the sixth day give one sheng of Qi-Ordering Decoction. When yangming and shaoyang are diseased together there must be diarrhoea. If the pulse is not defeated, the case is favourable; defeated means loss, when the two overcome each other, and this is called defeat. A slippery, rapid pulse means retained food; it should be purged, and Major Qi-Ordering Decoction is suitable.

Textual notes

Readings follow the base edition; significant variants are recorded here.
