Literal
燥 zào is parched, dried by heat; 屎 shǐ is excrement. The compound names not the ordinary costiveness of a dry bowel but stool that heat has dried into knots in the yang brightness interior — the thing the chapter feels for through the abdominal wall, tests for with a small dose of medicine, and then attacks with a purge.
Shuowen

In brief

Stool dried hard by heat in the yang brightness interior. The chapter treats it as a question to be settled before purging, never assumed: a small dose of the minor qi-settling decoction is given as a test, and if wind passes below, dry stool is there and may be attacked; if it does not, the stool is hard only at the head and the purge is withheld. Its signs are five or six days without stool, pain around the navel coming in bouts, restlessness, and — where heat has consumed the stomach — an inability to eat. Where it is present the great qi-settling decoction is used; where the stool is merely hard, it is not.

Senses in the classical corpus

1
The test: where stool has not passed for six or seven days and dry stool is suspected, a small dose of the minor qi-settling decoction is given first. If wind passes below, dry stool is present and may be attacked; if not, the stool is hard only at the head and must be left alone.
The signs in the patient's own words: five or six days without stool, pain circling the navel, restlessness coming in bouts. The chapter reads these as dry stool already formed, and does not wait for the pulse to say so.
Where the patient cannot eat, the chapter concludes that five or six pieces of dry stool are in the stomach; where he can eat, the stool is merely hard. The appetite thus becomes a sign of whether matter or only dryness is present.
After a heavy purge, six or seven days without stool with fullness and pain: old food had not been cleared, and the dry stool has formed upon it. The purge is repeated, because the matter was never wholly removed.

In yangming disease with tidal fever and slightly hard stool, Major Qi-Ordering Decoction may be given; if the stool is not hard, it is not given. If there has been no stool for six or seven days and dry dung is feared, the way to find out is to give a little Minor Qi-Ordering Decoction. If after it enters the abdomen gas passes down, there is dry dung and it may be attacked. If no gas passes, only the first part is hard and the rest will be loose; it must not be attacked, for attacking brings fullness and inability to eat. If the patient wants water, giving it causes retching; if fever comes afterwards, the stool will be hard again and scanty, and Minor Qi-Ordering Decoction harmonises it. When no gas passes, it must by no means be attacked.
