The Pattern of Cold Damage

Shāng hán lì dì sān

Cheng Wuyi's own treatise on cold damage, not a sequence of questions but a single argument carried through fifty-eight paragraphs. It sets out the four seasons and their proper qi, and then the qi that comes at the wrong time — warm in winter, cold in spring — and names the illnesses that follow: seasonal epidemic, cold damage, warm disease, wind warmth. It explains how cold enters, where it lodges, how it changes, and why the same cause gives a different illness in a strong man and a weak one. The six channels are then taken one by one, each with the day on which it is affected, the signs by which it is known, and the point at which it may be resolved by sweating or must be resolved by purgation. The last paragraphs warn against treating what is not yet settled and against treating what is already past.Because the base text prints this chapter as continuous prose, its paragraphing follows the sense rather than the question; the rule is stated in the notes below.

The Yin Yang Da Lun says: in spring the weather is mild, in summer hot, in autumn cool, in winter cold and sharp — such is the order of the proper qi of the four seasons.

In winter the cold is severe and all things hide deep. The wise man keeps himself enclosed, and so is not hurt by the cold; he who braves and offends it has what is called cold damage.

Injury by the qi of the four seasons can all cause illness. Cold damage is called a poison because it most becomes a killing and harsh qi. One who is struck and falls ill at once has what is named cold damage; one who does not fall ill at once has the cold poison stored in the flesh and skin, and in spring it turns to warm disease, in summer to summer-heat disease. Summer-heat disease is the extreme of heat, heavier than warm disease.

The Inner Canon says: before the summer solstice it is warm disease, after it summer-heat disease. Warm and summer-heat disease both come from injury by cold, and so the imperial physicians group them under cold damage. Hence toiling men suffer warm-heat disease in spring and summer; it all comes of braving the cold in winter, and is not seasonal epidemic qi.

Seasonal epidemic means that spring, which should be warm, turns bitterly cold again; summer, which should be hot, turns cool; autumn, which should be cool, turns hot; winter, which should be cold, turns warm. This is qi arriving out of its season. Hence within one year the illnesses of old and young are much alike — this is seasonal epidemic qi.

When winter has a warmth out of season it is named winter warmth. The poison of winter warmth differs greatly from cold damage. Winter warmth also comes earlier and later, piling one on another, and has its light and heavy cases; the treatment differs, and the signs are given in a later chapter.

After the beginning of spring, if there is no violent cold and no ice or snow, yet men fall ill with violent heat, this belongs to the spring yang qi rousing the hidden cold of winter and turning it into warm disease. From the spring equinox to the autumn equinox, violent cold from heaven is always seasonal cold epidemic. In the third and fourth months cold may come when the yang qi is still weak; checked by it, the heat of the illness stays light. In the fifth and sixth months the yang qi is already full; checked by cold, the heat is heavy. In the seventh and eighth months the yang qi has already declined; checked by cold, the heat is slight. The illness resembles warm and summer-heat disease, but its treatment is distinct.

Thus the warmth of spring becomes the heat of summer, and the anger of autumn becomes the severity of winter. Therefore after the winter solstice one yang line rises and one yin line falls; after the summer solstice one yang qi descends and one yin qi ascends.

Men of today who take cold damage sometimes do not treat it early, sometimes treat it wrongly, sometimes let many days pass and only call the physician when they are exhausted; and the physician does not treat in due order, so the treatment misses the illness. All should weigh the case and frame the prescription on the spot, and none would fail. Now I have gathered Zhang Zhongjing’s old discussions, recording the signs, the pulse, the voice and complexion, and the true prescriptions matched to the illness that have shown divine effect, to meet the urgent needs of the world.

Again, the land is warm or cool, high or low, and the nature of things hard or soft, their food and dwelling also differ. Therefore the Yellow Emperor raised the questions of the four quarters, and Qi Bo set out the powers of the four treatments, to instruct later worthies and open what they had not awakened to. The physician at the sickbed must examine both.

If cold strikes both channels at once: on the first day the greater yang takes it and is diseased together with the lesser yin, giving headache, dry mouth, restlessness with fullness and thirst. On the second day the yang brightness takes it and is diseased together with the greater yin, giving belly fullness, body heat, no wish for food, and delirious speech. On the third day the lesser yang takes it and is diseased together with the terminal yin, giving deafness, drawn-up scrotum and cold limbs, with food and drink unable to enter and no recognition of people; such a one dies on the sixth day. If the three yin and three yang, the five viscera and six bowels are all diseased, then the ying and wei no longer move and the viscera and bowels are blocked, and death follows.

When the seasonal qi is discordant one should speak early, trace where the evil came from, and while it is still in the interstices treat it in time; rarely does such a case fail to heal. The sick man bears it for several days and only then tells, when the evil has entered the viscera and is hard to control. This is the essential point for a household that has a sick man to consider beforehand.

In making decoctions one must not avoid dawn or night; feeling the illness, one should treat it at once without waiting for morning or evening, and it heals easily. If there is any delay, the illness transmits and changes, and though one wish to treat it, strength will not serve. Medicine must be taken by the method; to indulge one’s own will and defy the teacher is not to be treated at all.

Cold damage mostly comes from wind and cold. When wind and cold strike the exterior, once they enter the interior they do not disperse. There is no case where the covering was rightly warmed and the evil did not disperse.

If one would attack without regard to the pattern, one should still first release the exterior and only then purge. If the exterior is released but the interior does not disperse, and it is not great fullness, with cold and heat still arising, the illness is not removed. If the exterior is released and the interior does not disperse, with great fullness and great repletion and hard bound dry stool, it may of course be cleared downward; even after four or five days it will do no harm.

Where yang is full and yin vacuous, sweating kills and purging heals; where yang is vacuous and yin full, sweating heals and purging kills. Thus how can divine elixir be wrongly used to sweat, or sweet flag wrongly used to attack? The treatment of vacuity and repletion lie a thousand li apart, and the hinge of fortune and misfortune answers like shadow and echo — how can it be lightly taken? Moreover, when cassia twig goes down the throat, the yang-full die; when Qi-Attack enters the stomach, the yin-full perish. The crux of life and death lies in an instant; watching a life cut short, one has no leisure to count the days.

This is the interlacing of yin and yang, vacuity and repletion; its signs are exceedingly subtle, and the contrariety of sweating, vomiting and purging brings disaster exceedingly fast. Yet the shallow and narrow in the art, muddled and ignorant of the source of illness, treat wrongly and cause the sick to perish, saying it was their lot; so that to this day wronged souls choke the road of the dead and corpses fill the wild fields. Does not the humane man grieve at this? When double contraction breaks out together, treatment has its order; releasing the exterior and attacking the interior are from the first not the same. Yet the obstinate and self-willed say that divine elixir and sweet flag should be drunk together, to release the exterior and at once clear the interior — clever words that seem right, but their principle is truly perverse. The wise man’s every move is always weighed with care; the fool’s every act is sure and swift. Can the changes of safety and danger be juggled with? Men of the world strive only for the glory of the moment and never see this ruin of overthrow; only the clear-sighted can quietly guard his root, taking witness from his own body — what need to seek afar?

In sweating, the decoction is taken warm. Though the prescription says three doses a day, if the illness is severe and does not resolve, one should shorten the intervals and finish the three doses in half a day; if the medicine is obstructed by the illness, the effect is felt at once. In heavy illness one watches a full day and night. If after one dose the signs remain, the same decoction should be made again; some will not sweat until three doses are taken. If no sweat comes at all, it is a fatal illness.

In seasonal illness, by the fifth or sixth day there may be thirst for water, yet not much can be drunk; one should not give it. Why? Because the heat in the belly is still slight and cannot dissolve it, and it only adds to the illness. By the seventh or eighth day, with great thirst, one should still give according to the signs, always leaving the man short of enough and never letting him drink his fill: if he can drink a peck, give five pints. If drinking brings belly fullness, difficult urine, wheezing or hiccup, it must not be given again. A sudden great sweat means he is cured of himself.

Textual notes

Readings follow the base edition; significant variants are recorded here.
