癰 yōng — the one disease the chapter treats twice, at the gastric duct and at the neck, and the word it uses to make its sharpest practical point. The graph holds 壅 within 疒: something dammed up under the skin. The chapter is not content with the name, and says plainly that two abscesses bearing the same name can be two different things, needing two different instruments.

《說文解字》:癰,腫也,从疒雝聲。「癰」讀 yōng、**本篇4見**:**胃脘癰二見、頸癰一見、癰氣一見**;**底本「癰」不从「雍」之省,各處同形**。

In brief

A gathering that has Ripened and is walled off: heat dammed at one place until it can no longer move on. Chapter 46 reads an abscess of the gastric duct off the neck pulse — counterflow and fullness there means heat has collected at the mouth of the stomach — and then uses an abscess of the neck to state a principle: the same name covers two grades, one still growing and one where blood has already gathered, and the grade and not the name decides whether one uses the needle or the stone.

The Emperor asks: when a person has an abscess of the gastric duct, how should one examine him? Qi Bo answers: to examine this one should read the stomach conduit. The conduit should be sunken and fine; sunken and fine means the qi is counterflowing, and where qi counterflows the ren ying pulse is exceedingly full. Exceedingly full is heat.

When greater yang governs the heaven the qi of cold descends upon it and the qi of the heart follows upward; fire then is bright and vermilion arises, and metal suffers calamity; cold and clear rise again and again, and where cold prevails the water freezes, so that the qi of fire is bright and high. The illness is heat and oppression in the heart, a dry throat and much thirst, snivelling and sneezing, a tendency to grief and many yawns. The hot qi moves recklessly and cold returns; frost falls out of season; there is much forgetting, and in the extreme pain in the heart. Earth then is moist, water abundant and spreading, cold comes as a guest, heavy cloud transforms, damp qi alters beings, water and drink store within, there is fullness in the middle and no eating, the skin is numb and the flesh callous, the sinews and vessels are not free, and in the extreme water swelling and a boil behind the body.
