Pinyin

Simplified
疟气
Literal
瘧 nüè, malaria; 氣 qi. The evil itself, as distinct from the disease it causes — the thing that lodges, travels, keeps pace or fails to keep pace, and answers when the defensive qi comes round. The chapter needs the word because it is explaining behaviour: it is not 瘧 that sinks and lodges, it is 瘧氣. The distinction shows up most clearly in the fifth answer, where the malarial qi follows the channels and networks and sinks inward, so that it breaks out only when the defensive qi answers it.

In brief

The malarial evil, spoken of as a thing that moves. It lodges at a depth, and that depth decides everything: shallow means it keeps pace with the defensive qi and the fit is daily; deep means it cannot, and the fit comes every other day or every few days. It is exuberant and vacuous by turns, and which it is at a given moment determines heat or cold, a restless or a quiet pulse.

How it has been translated

Every rendering below is a decision about which reader is being addressed. There is no neutral option.

Wiseman & Fengmalarial qiThey keep the construction 病名 + 氣, which is how the chapter builds all its qi terms.
Unschuldmalaria qiUnschuld keeps it literal, as he does for the other compound qi of the chapter.
This sitemalarial qiWe keep qi rather than evil here: the chapter has 邪氣 available and uses it when it wants the evil, so 瘧氣 is doing something more specific — naming the qi of this disease, the thing whose depth and pace the whole chapter is about.
