腸 cháng — the intestine; 覃 tán — to extend, to reach far, to spread. The mass that grows by spreading along the intestine, outside it.
Shuowen
《說文》:腸,大小腸也;覃,長味也,引申為延及。「腸覃」本篇2見,與石瘕對舉,一在腸外,一在胞中。

In brief

The first of the two abdominal masses, and the one that moves. Cold qi lodges outside the intestine and wrestles with the guarding qi; the qi can no longer nourish, something ties and binds, evil qi rises and a fleshy polyp grows. It begins the size of a hen’s egg and enlarges until the belly looks like a pregnancy; in long cases it lasts for years, feels hard under the hand, shifts when pushed, and the menses come on time. That last clause is the whole diagnosis: a mass outside the womb leaves the womb’s calendar intact, and it is exactly what separates 腸覃 from 石瘕, which sits in the womb and stops the menses. The name is thus a description of behaviour — a thing that extends along the gut, outside it — rather than of shape.

What of intestinal swelling? Qi Bo said: Cold qi lodges outside the intestines and knots with the guarding qi, so that qi cannot run freely. Something becomes tethered, a mass clings within and does not leave, evil qi arises and a polypous growth forms. At first it is the size of a hen’s egg; it gradually enlarges, and when formed it looks like a pregnancy. In long cases it lasts beyond the year. Press it and it is hard; push it and it moves. Menstruation comes on time. That is its sign.
「腸覃」本篇2見,寒氣客于腸外而生瘜肉,按之堅、推之移、月事以時下,與石瘕之在胞中者正相反。
