手 shǒu — the hand; 毒 dú — what is noxious, what bites. Together: the hand whose touch is injurious, the reverse of 手甘, the hand whose touch heals.
Shuowen
《說文》:毒,厚也。害人之艸,往往而生。从屮从毒。醫書以「毒」言藥之烈、氣之厲,此篇則以言手:手毒者按龜龜死,手甘者按龜復生,是手自有厚薄之性。

In brief

Hand poison is the chapter’s test for the man who should be set to pressing accumulations. After listing who is fit for what — the clear-eyed for colour, the sharp-eared for sound, the quick-tongued for disputation, the steady hand for needles and moxa, the gentle for stretching and guiding — Huang Di adds the last office: 爪苦手毒,爲事善傷者,可使按積抑痺. The hard-nailed and hand-poisoned man, the one who breaks what he handles, is the man for pressing masses and suppressing obstructions. The test is a tortoise: put it under a vessel, press the vessel for fifty days, and 手毒者 the tortoise dies; 手甘者 it comes back as it was. The passage is the Lingshu’s one experiment, and its point is that the capacity is in the hand and not in the intention — which is why the chapter can assign offices at all.

Hence it is said: to the right man speak; to the wrong man do not transmit — this is what it means. The poison-handed can be made to test by pressing a turtle: put the turtle under a vessel and press on top, and in fifty days it dies. The sweet-handed one comes back as before.
「手毒」本篇2見,在官能末段,以按龜生死驗手之毒甘,與上文各得其能相足,故引全段。
