Taiyin Disease

Biàn tài yīn bìng mài zhèng bìng zhì dì shí

The taiyin chapter, nine passages, and the chapter of cold below. It opens with the outline: fullness of the abdomen with vomiting, food that will not go down, diarrhoea growing worse, and pain in the abdomen coming of itself from time to time. Purge it and there is hardness and binding below the heart — the one prohibition on which the chapter turns, since taiyin is a disease of cold and emptiness and purgation drives the yin evil inward. What follows is a chapter of distinctions. Wind in taiyin with restless aching of the limbs, and a faint yang, rough yin pulse that is long, which means recovery is near. The hour of resolution, from hai to chou, when the earth of the spleen is at its height. A floating pulse, where sweat may after all be induced with Cinnamon Twig Decoction. Diarrhoea without thirst, which belongs to taiyin and its cold viscus, to be warmed with decoctions of the Four-Rebellion kind, set against the thirst of shaoyin and its cold in the lower burner. And the seventh or eighth day, when sudden irritation and ten or more stools a day end of themselves, because the spleen household is replete and the rotten matter is leaving. The chapter closes on the two formulas that carry taiyin back toward yangming: Cinnamon Twig plus Peony for fullness with intermittent pain, and Cinnamon Twig plus Rhubarb where the pain is one of great repletion — with the caution that where the pulse is weak and the stool passes freely, the rhubarb and peony must be reduced. Cheng Wuyi's commentary is quoted selectively, marked 成注(節).

The disease of taiyin is fullness of the abdomen with vomiting, food that will not go down, diarrhoea growing worse, and pain in the abdomen coming of itself from time to time; if it is purged, there will surely be hardness and binding below the heart. Taiyin belongs to the spleen; when the spleen is empty its transformation fails and cold dampness settles, which gives rise to these signs.

Diarrhoea without thirst belongs to taiyin, because the viscus has cold; it should be warmed, and decoctions of the Four-Rebellion kind are suitable.

In cold damage with a floating, moderate pulse and warm hands and feet of themselves, the disease is tied to taiyin, and taiyin should make the body yellow; if urine flows freely, it cannot turn yellow. By the seventh or eighth day, though there is sudden irritation and ten or more movements a day, the diarrhoea will stop of itself, because the spleen household is replete and the rotten matter should leave.

Textual notes

Readings follow the base edition; significant variants are recorded here.
